PCD BENEFICIARY INFORMATION FORM

Dear Muscowpetung Band Members,

The Chief and Council have been working diligently to develop a simplified process for membership to make the Per Capita Distribution as easy as possible. We are pleased to provide an update to membership regarding the process for submitting information for the current PCD’s. We have supplied the form as a downloadable/printable form that can be printed and filled – this form can be found below. These completed forms can be submitted through the following formats:

– In-person with our PCD Coordinator

– Send it through email

– Mail in

For anyone that has already submitted information through the online form, please refrain from re-submitting – as all forms previously submitted are in possession (online forms, email, and mail-in). An email confirmation has been sent to those forms that have been submitted. As we work through the forms, please be patient, we will be in contact to request any missing required information that may be required.

For membership that have yet to submit their information for the PCD, please review the documents provided below, as they provide further information for the PCD process.
