Legend (Rating Criteria):
5 – Expert (student performs all tasks proficiently and independently).
4 – Competent (student performs efficiently in an effective and efficient manner).
3 – Progress Acceptable (performance is usually effective and but not always).
2 – Needs Improvement (progress in performance is too slow to judge satisfactorily;
task performance is not most of the time).
1 – Progress Unacceptable (no progress in performance has been demonstrated, and
or performance is consistently ineffective and inefficient).
Partograph is a useful tool for monitoring the progress of labor. It is use to avoid
unnecessary interventions so maternal and neonatal morbidity are not needlessly
increased, to intervene in a timely manner to avoid maternal and neonatal morbidity or
mortality and to ensure close monitoring of the woman in labor.
USES: 2. Assessment of maternal well being
1. Assessment of progress of labor pulse, temperature, blood
cervical dilatation, pressure
contractions and alert and urine voided
action lines 3. Assessment of fetal well being
fetal heart rate and pattern color
of amniotic fluid
Materials needed: Partograph chart
Parts of the Partograph :
1. Upper colored portion – is where the progress of labor is plotted.
2. Lower portion – is where observations of the maternal and fetal well-being are
written.

1. Plot cervical dilatation when labor is in active phase
2. Monitor and record vital signs and cervical dilatation every
4hours or frequently as necessary
3. Monitor and record every hour:
- FHT, frequency, intensity and duration of contractions,
woman’s mood and behaviour.
4. Monitor and record other findings:
- vaginal bleeding
(0,+,++,+++)
- time membranes ruptured
“I” if membranes are intact
- color of amniotic fluid
“C” membrane are ruptured and amniotic fluid is clear
“M” amniotic fluid is stained
“A” Amniotic fluid is absent
“B” Amniotic fluid is bloody
- Urine voided (yes, no)
