Great question. Speaking specifically in the context of managing MCP servers , my understanding is that these two solve different, complementary problems at different layers of the stack.

Azure API Management (APIM): In this context, APIM acts as the "North-South" AI Gateway focused on stateless corporate governance. It's the main front door for enterprise-wide concerns like end-user Authentication (OAuth) , token-based Rate Limiting , Semantic Caching , and integrating security policies like Azure AI Content Safety.

microsoft/mcp-gateway: This is a specialized "East-West" Session Gateway designed to run inside a compute environment like Kubernetes. Its key feature is "session-aware stateful routing". Unlike a traditional gateway, it's built to understand the MCP protocol body (JSON-RPC) to read the session_id and ensure all messages for that session are pinned to the same backend MCP server pod. This is critical for horizontally scaling stateful MCP operations.

In short: APIM manages the stateless corporate governance (the front door) , while mcp-gateway manages the stateful protocol routing (the session-aware interior) needed for scaling in Kubernetes.
