I would love to see a bit more added to describe what is allowed within Middleware Operations and in what circumstances. For example:

• How should middleware accommodate streaming Applications?
• Is a Middleware allowed to add Headers after calling next?
• May a Middleware buffer the request or response Streams?
• Must it alert the user in some way?
• and so on and so forth.
