Major changes:

• ctrlcctrlv@e9fbbcb (Add ability to set org name, and issuer information of Certificate Authority.)
• ctrlcctrlv@29eb639 (Singularize --domain and --ip-address, make it so these flags can be repeated but remove the "list" method used here.)
• ctrlcctrlv@a108699 (Add --mac-validity, off by default in minica2)

Because this fork adds several changes likely to be controversial, and which break backwards compatibility, I did not open PR's.
