Proposed improvement

Add a "reason" field that the uploader must fill before file upload starts.
This new feature would be behind a config option, disabled by default.
This field would be logged along with other upload metadata and optionally stored in DB along with the upload.

Why?

For auditability concerns, when using plik in sensitive environments or as a file gateway between sensitive and non sensitive environments.
