This form shall contain a school or a division's request for data correction in the LIS. These data include learner enrollment records wherein the grade/year and/or enrollment status in previous school years were not cleaned up, hence rendered "Not Eligible" for enrollment in SY 2013; and the deactivation of duplicate LRNs. Please send this request to Mr Deo Genito at [email protected], copy email address of your respective Division Planning Office.
