USER PAGE - 1 Date: Doctor : Date picker SEARCH dropdown

NB: select a time schedule. Put one search @ top side .. DATE search. Screen 2 If user clicked on free time slot next step :- show 2 radio buttons. Like 1) New patient 2) Registered Patient

NB:- if some one clicked n new patient radio button show the following fields

Screen 3 (term 1) *Title : *First name : *Last Name: Address: Town/city: Post box: *Mobile No: *Email: SUBMIT dropdown

NB:a) keep mobile number and email address as unique. b) Mandatory fields :- title, First name, last name, mobile number and email id. c) After successful submission send a 4 digit code to his/her mobile number.

Screen 4 (confirm activation code)

If it is valid --- just confirm his/her appointment request and lock the selected time slot in time table. And also sent a mail to the patient/user with his auto generated patient Id number for future reference.

a) Appointment calendar :- same like front end. Admin can block and unblock appointment schedules. b) Appointments( EDIT/DELETE/SEARCH) :- VIEW appointments 50 in first page (implement paging) SEARCH.
