Nature of works to be carried out, include areas. Method Statement Number and Revision. Fully describe works to be carried out and location. Detailed method of working including provision of sketches to show position of any cranes, concrete pumps, crane mixers, lorries etc and any other information relative to working safely and in an environmentally sensitive manner. Include details of any special PPE required. Either refer to programme for the works or give dates/times if applicable. Must clearly identify time, duration and sequence. Skills cards (CSCS, EUS etc) Plant operatives. Training Certification i.e. PASMA, HILTI, Confined Space, Abrasive Wheels Training, Supervisors Training, First aid Certification. The type of plant and equipment, how we ensure it is fit for purpose. Necessary certification for plant, electrical equipment etc Describe arrangements for recovery / rescue, special first aid and temporary re-routing or changes to access provisions. Name supervisor responsible and give title. Deputy to be named to cover absenteeism. State method of passing information to operatives, confirm toolbox talks. Confirm mechanism for approval and alteration of agreed method statement. Mechanism for monitoring compliance to agreed method statement.
