Requirements
READMISSION

1. Long brown envelope with your name


2. Printed OSH Application Form
3. 2x2 ID picture with name written at the back
4. True copy of grades of two previous semesters or certified CRS print-out with general
weighted average (GWA)
5. Undergraduate students:
1. Income Tax Return
1. Father OR Mother;
2. Father AND Mother (if both are working);
3. Bureau of Internal Revenue (BIR) Certificate of Tax Exemption (if both
parents are not working OR one parent is not working)
6. Clearance (financial and disciplinary) from former residence hall manager
