AGREEMENT
I understand that my child/ward is participating to the club 9-day community service in the
church procession every afternoon from April 21-30, 2018 and fund raising activity from April 23-May 5,
2018 which will be lent for club works and projects.

I understand that the 4-H members are required to pay a membership fee of PhP 20.00 that is
based on the constitution and by-laws of the 4H club.

I understand that participating in club activities do not guarantee an accident free environment.

I understand that all the activities of the 4H club are for the training of its member which will
benefit and equip them skills, learnings, and experiences for their development.

I understand and agree to all aforementioned detail in which I fully support my child/ward in
his/her active participation in the 4H club.
