Ravi Rathnayaka submitted an application to upgrade his existing Sampath Classic credit card to a Sampath Gold credit card. He included with his application a signed form, copy of his identity card, employer confirmation letter, and copies of three salary slips. He also attached a copy of his request to cancel his existing Sampath Classic credit card via Sampath Wishwa.
