Five students from section B of a bridging program broke five test tubes during a chemistry lab experiment on June 4, 2019. They were heating iron and sulfur in a test tube over a Bunsen burner, which caused an unwanted chemical reaction that broke the test tubes. The students took responsibility for the incident and offered to pay to replace the five broken test tubes.

Incident Report

June __,2019

To Tagum Doctor’s College Laboratory

We, students of section B of the bridging program, broke five test tubes
during Chemistry Laboratory experiment last June 4, 2019(Tuesday). The chemical
reaction of Iron and Sulfur inside the test tube when being directly heated on a
Bunsen burner is reason behind the unwanted breaking of the test tube.
We are responsible for the expenses of buying new five test tube as an
exchange. Thank you and God bless.
