JOINT AFFIDAVIT OF LEGITIMATION

legal age, husband and wife, respectively, after having been duly sworn in accordance with law,
depose and state:

(1) That we are the biological parents of ______________________________, who was

attached birth certificate, marked as Annex A;

(2) That at the time of the child’s birth, we, the biological parents, were not yet married to
each other;

(3) That at the time the child was conceived, there was no legal impediment for us to
contract marriage;

(4) That at the time of marriage, we were both single;

(5) That we were married on ____________________ in ____________________, as


evidenced by the attached Marriage Certificate, marked as Annex B;

(6) That by virtue of our subsequent marriage, the said child is now legitimated by operation
of law;

(7) That the father acknowledges the paternity of the child, and requests the name of the
child to read as ______________________________ by virtue of his/ her legitimation by
our subsequent marriage;

(8) That we are executing this Affidavit to attest to the truth of the foregoing, for purposes of
complying with the requirements in applying for the legitimation by subsequent marriage
of our child, and for all legal intents and purposes.
