AFFIDAVIT OF LEGITIMATION

KNOW ALL MEN BY THESE PRESENTS:

We, SPS. ROSEMANE and DANIEL PADILLA, both of legal age, Filipino citizens, and residents of Brgy.
Lingsat, City of San Fernando, La Union, Philippines, after having been sworn in accordance with law, hereby
depose and state the following:

1. That we are the biological parents of BIMBY PADILLA, who was born on February 14, 2019 at
Brgy. Lingsat, City of San Fernando, La Union and was registered at the Office of the Civil
Registrar of the City of San Fernando, La Union;
2. That during the birth of said BIMBY PADILLA, we were not yet married but we did not have any
legal impediment to contract marriage at that time;
3. That subsequently, we got married on July 17, 2019 in the City of San Fernando, La Union;
4. That by virtue of the subsequent marriage, we legitimize our child BIMBY PADILLA as we were
not disqualified by any impediment to marry;
5. As such, we are executing this Affidavit to attest to the truth of the foregoing facts and for any
other legal purpose that this Affidavit may serve.

IN WITNESS WHEREOF, we have affixed hereunto affixed our signatures this 4th day of November 2019
in the City of San Fernando, La Union, Philippines
