AFFIDAVIT OF DRIVER

I, WALT GEF F. CALAGO, of legal age, Filipino, single and a resident of Brgy.
Dr. Jose Rizal, Katipunan, Zamboanga del Norte, under oath, hereby depose and say,
That:

1. I am the driver of a vehicle registered under my name, a certain Brand


New Ford Ranger with Plate No. NCQ 9395, Engine No. P5AT2578677
and Chassis No. MNCUMFF50JW810920 vehicle (vehicle for brevity);

2. On January 13, 2020 at around 9:00 o’clock in the evening, while I was
driving the vehicle along the national highway of Polanco, Zamboanga
del Norte, I encountered an over-speeding truck which was about to
overtake another car, thus, I swerved my driven vehicle to the right to
avoid the said truck, unfortunately, I accidental bumped into a tree
beside the road;

3. The said accident caused damages to the vehicle, to be specific: broken


front bumper;

4. The damages sustained by the vehicle entail immediate replacement


and repairs particularly for the replacement of the front bumper; and

5. I am executing this Affidavit in order to attest to the truth of the


foregoing facts and for all legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand and affixed my


signature this 17th day of January, 2020 at Dipolog City, Philippines.
