I, LOUI B. SANCHEZ, Filipino, of legal age, and a resident of Arana St, Brgy. Sta Cruz Naga
City, after having been duly sworn to in accordance with law, hereby depose and say:

1. That I am the respondent in the above-titled case;

2. That I have caused the preparation of the above answer and have read and
understood the same;

3. That to the best of my knowledge and belief, the contents thereof are true and
correct and/ or based on authentic records;

4. That I have not commenced any other action or proceeding involving the same
issue in the Supreme Court, the Court of Appeals or any other tribunal or
agency;

5. That to the best of my knowledge, no such action or proceeding is pending in


the Supreme Court, the Court of Appeals or any other tribunal or agency;

6. That in the event that I learn that a similar action or proceeding has been filed
or is pending before the Supreme Court, the Court of Appeals or any other
tribunal or agency, I hereby bind myself to report the said fact within five (5)
days therefrom to this Honorable Court.

7. That I executed this verification/certification to attest to the truth of the


foregoing facts and to comply with the provision of Adm. Circular No. 04-94
of the Honorable Supreme Court.
