Application for the grant, renewal, transfer or amendment of a licence in Form LS- 2 to transport compressed gas in a
vehicle

3. Name of the compressed gas proposed to be transported (Please state whether
flammable, corrosive or toxic).

4. Chemical name of compressed gas to be transported in the vehicle


5. Water capacity of the pressure vessel and net weight of the compressed gas
proposed to be transported.

6. Number and date of approval of the mounting drawing of the vehicle by the Chief
Controller

7. Does vehicle (described above) fully conform to the requirements laid down in
the rules and the mounting drawing approved by the Chief Controller.

8. Full postal address of the place where the vehicle will be normally stationed.

I/We declare that the particulars given above have been checked up by me/us and are correct. I/We undertake to
transport compressed gas in the vehicle particulars of which are given above, in accordance with the provisions of the
Indian Explosives Act, 1884 and the Static and Mobile Pressure Vessels (Unfired) Rules, 2016 framed there under and
any other law or rule for the time being in force. I/We understand that any contravention of the said Act and the rules
framed there under is punishable with imprisonment for a term which may extend to two years, or with fine which may
extend to three thousand rupees, or with both.

NOTE:
DOCUMENTS REQUIRED TO BE SUBMITTED WITH THIS APPLICATION

(1) Four copies of drawing approved under Rule 35 and as-built vessel design drawing endorsed by Inspector
(required only for grant or transfer or amendment with modifications in vehicle or vessel).

(2) Safety certificate required under Rule 43 along with test and inspection certificate of the vessel in original
plus two photocopies (required only for grant or transfer or amendment).

(3) Original valid licence, if the vehicle was previously licensed (required only for renewal, transfer or
amendment).

(4) Requisite amount of fee for the grant, renewal, transfer or amendment of licence as per schedule 1.
(5) Copy of the registration certificate of the vehicle issued under the Motor Vehicles Act, 1988 (59 of 1988)
and weighment slip in respect of mobile pressure vessels for transport of compressed gas (required only for
grant or transfer of licence).
