having been sworn in accordance with law, hereby depose and state
thus:

1. I am respectfully informing and requesting the Bureau of


Customs, Account Management Office for the amendment and
change of our principal address from:

4. I am executing this Affidavit of Change of Address in order to


amend and change our business address indicated in our Certificate of
Registration in Bureau of Customs.
