DATABASE SYSTEM
Assume that you are an employee of a consultancy company that specialized in the
analysis, design and implementation of database systems. A client has recently approached
your company with a view to implementing a database system, but they are no familiar with
the development process. Provide a report explanation for your client.

Task 1: Fact Finding
Interviews
Interview is a very important data gathering technique as in this the analyst directly contacts
system and the potential user of the proposed system. Interviewing is the most commonly
used, and normally most useful, fact finding technique. This method is used to collect the
information from groups or individuals. Analyst selects the people who are related with the
system for the interview. In this method the analyst sits face to face with the people and
records their responses. Using the interviewing technique requires good communication
skills for dealing effectively with people who have different values, priorities, opinions,
motivations, and personalities.
Example of the question:
1. What do you expect from this system?
2. Who will use this system frequently?
3. Do you think this system will make your work more organise?
4. What is the scope for this patient information system?
Questionnaire
In this method, the analyst can collect data from a large group. The questions are simple,
clear and to the point. This method is very useful for attaining information from people who
are concerned with the usage of the system. Questionnaires could be open-ended
questionnaires and closed questionnaires. Open-ended are used to learn feelings, opinions,
general experiences on process detail or problem. Closed questionnaire is asset of
prescribed answers are used and specific response have to be selected. In this case, the
questionnaires are distributed to the user specifically nurse, doctor, medical assistant and
etc.

This is a skill that analysts have to develop. The analysts have to identify the right
information and choose the right person and look at the right place to achieve the objective.
The analyst should have a clear vision of how each departments work and work flow
between them and for this he should be a good observer. In analyst may observe the
unwanted things as well and simply cause delay in the development of the new system.
There are 3 types of observation which are participant observation, non-participant
observation and time allocation studies. In this situation the analyst will observe in the
registration department for the detail of the patient.
Record View and inspections
Basic records like the reports, bills, policy manuals, regulations, standard operation
procedures can be used for better understanding the system. The information are related to
the system is published in the source like newspaper, magazines, journals and documents.
The record reviews help the analyst to get valuable information about the system and the
organization.

Introduction
The patient information record is a multifunctional document that is used to communicate
and document information about the patient. Patient information records are a cornerstone in
the quality and efficiency of patient care during the hospitalization and in subsequent follow-
up visits, as they can provide a complete and accurate of treatments, patient results and
future plans for care. Despite the importance of medical records to high quality and efficient
care, management of the hospital, has not been a priority. The patient information records
function is to support by extensive use of information technology, patient information records
are generally inadequately supported and poorly managed. Although there are some
exceptions with new open source patient information records systems becoming available,
these are not yet widely used.
In this study, we report on a re-engineering process to improve patient information records
management. Before this, the hospital used a paper-based registration. So we are
approached by the client to develop with a computer database program to assist patient
information search. We used a quality improvement approach, which involved hospital
management, physicians and medical record staff and evaluate the impact of the new
system on patient information system accessibility and completeness, as well as changes in
physician satisfaction with the medical records and patient registration of quality
improvement in strengthening medical records management, a foundational aspect of health
system.
Problem Statement
The inquiry of the patient information system is to keep track and records about the patient
information in one of the hospital in Shah Alam that used a paper based before this. This
system supports accessing the previous visit histories of the patient, search for patients and
other properties. This hospital has 250 beds and 150 of employees with 10 physicians.
Scope of System
The patient information system is to support the registering of the patients. The users of this
patient information system will search for the patient by using their name, admission date,
discharge date and etc. Users are also can view the previous visit histories of any patient.
This system can maintain the list of doctor in the hospital also can maintain the list of the
beds or rooms available in the hospital. This system also can categorised patient into ‘In
Patients’ or ‘Out Patient’.
