The Composer/Musician, who owns the copyrights to the music track grants permission to
Your Name/Production Company to use the work listed below on the production.

TERMS OF AGREEMENT

As compensation for the use of the Music Track(s)/Materials, the Composer will receive the
following Payment/Credit.

The Production Company has the right to alter or modify the material in any manner, at
its sole discretion. Additionally, the Production Company may at any time elect not to use
the materials, in which case neither party will have any obligation to use.

AUTHORISATION
The Composer/Musician owns the sole and legal copyright to the described work and
hereby agree to the use of this work in the promotion, production and distribution of
the film without further compensation. I agree to timely execute and deliver any additional
documents which may require to evidence its rights. I also understand that any and all
proceeds from the film are the property of the filmmakers.
