OUTPATIENTS

PROCEDURE

1. Patients register to the front officer for medical record and identity
2. Ask for the Identity card, insurance and referred letter (if available)
3. Waiting for the name called by the nurse
4. Have a consultation with specialist

Patient : hello my name is Ms. Rachel I want to register my self in outpatient list in
internal medicine for today
RM Officer : okay, let me check your identity first mam, could you please give me your
identity card, insurance and referred letter from primary health care ?
Patient : sure, this
RM Officer : hmm, I’ve checked your medical record but never been in the list for the
routine patient in this hospital. Have you ever went or seeing a doctor to the other hospital ?
Patient : no, this is the first time in my life going through all this.
RM : okay, wait for a moment. Here is your waiting list and please queuing right
there and the nurse will call out your name immediately.
Patient : oh thank you so much
RM : you’re welcome mam



Nurse : ms. Rachel!
Patient : yup! Im here
Nurse : please come inside and have a sit Mam
Doctor : hello good morning, I’m your internist doctor in charge for today, Ms. Rachel right ?
Patient : hi good morning doc, I am Rachel. Im having a really bad headache, please help me
with some medicine doc
Doctor : oh I’m sorry to hear that, first of all how long have you feeling it ?
Patient : it started about 5 days ago, but it getting worse and worse, I even feeling dizzy when I
woke up
Doctor : ok tell me exactly where the pain is? Could you please point it out?
Patient : it feels from around my neck to shoulder and right hand. Sometimes it feels so heavy
and dizzy.
Doctor : do you have any other symptoms? Like fever, nausea or even vomit?
Patient : no, the only thing I feel is a terrible headache.
Doctor : what are you doing to ease your symptom? Do you take any medication ?
Patient : I’m feeling better after I took Paracetamol, but it didn’t last long Doc.
Doctor : okay i’ll prescribe you a combination therapy for now, if it doesn’t help you latter we’re
going to do full body examination. Here it is you can go to the front door and ask by the nurse where is
the pharmacy room.
Patient : thank you doc.
Doctor : my pleasure , hope you get better Ms. Rachel
