1. Outline of breakdown
After the pilot boarded and while approaching a mooring buoy, the main engine cam shaft became
incapable of switching from ahead to astern operation. Switching was also not possible by way of C/R
control either. Although control was switched to the engine-side, the control selector lever (ordinary
/emergency switching lever) on the engine-side was stiff and switching could not be accomplished.
After shifting to a safe ocean area by way of W/H control and then repairing the three-way valve,
ahead – astern operation testing was again carried out on the engine-side. After confirming that opera-
tion was normal, port entry was carried out after performing test runs by way of W/H control.
2. Causes
Due to failure of the three-way selector valve POS91, operation was not possible. After checking the
availability of spare parts, overhaul inspection was carried out and the following was confirmed:
1) The plug valve seat was broken.
2) The spring (exterior) length was 5mm shorter than the spares.
3) All O-rings were hardened.
4) A portion of seal ring was broken.
3. Repair
Although the air cylinder blow valve was operated so that the cam shaft reversing booster could be
controlled manually, as switching could not be accomplished, a check of the air system revealed op-
eration failure of the three-way selector valve POS91, and it was replaced with a spare.
<About the three-way valve POS91>
1) According to the vessel’s history, the valve had been replaced during maintenance 7 years ago.
2) The three-way valve in question was a Swedish MECMAN model which discontinued 5 years ago,
and spares were not possessed by Mitsui Engineering & Shipbuilding Co., Ltd. (same for POS92,
93)
3) Mitsui Engineering & Shipbuilding Co., Ltd. is recommending replacement with an alternative
article at time of docking. 3 days are required for replacement.
