Labor and Delivery Report

The nurses' notes document a patient's visit to the hospital. The 37-week pregnant patient was experiencing regular abdominal contractions and pain radiating to the pelvis, with a slight bloody mucus discharge. Her vital signs and initial assessment were taken. To help with the pain, the nurse instructed comfortable positions, applied a perineal pad, started an IV fluid, and inserted a catheter. The patient's contractions reduced slightly from a 6/10 to a 4/10 pain level before she was transferred to another hospital for further care.
