Thank you for your interest in Head Start School System, our Registration Forms for admission
are available online and can be downloaded to be filled out manually, or can be submitted online
through our website as per your convenience. You may also visit the branch where you can
receive a manual Registration Form which you can fill out on the spot there.

Registration Schedule:

Saturday, 29th January 2011 (9:30 a.m. and 4:30 p.m.)


Monday, 31st January 2011 (2:00 p.m. and 4:00 p.m.)

Requirements: Please bring along the following for registration in both cases downloaded or
submitted online:

Birth Certificate (those made in the last six months will not be accepted).
Four passport sized photograph.
Progress report of the previous school (if any).

PLEASE FOLLOW THE STEPS BELOW:

A. Downloaded Forms:

1. Please take a printout on an A4 sized sheet & fill in all details by hand in block letters (all
areas are mandatory).
2. Please bring the form to the concerned branch where you wish your child to be registered,
as per the Registration Schedule above
3. Submit the form along with a registration fee of Rs. 500/- (please collect the receipt).
4. Receive a slip for test date/time.

B. Forms Submitted Online:

1. Please fill in all the fields as they are all mandatory.


2. Click to submit
3. You will receive an email informing you about your registration ID and confirming all the
information you entered online, therefore please be careful to enter your valid email
address on the form.
4. Please visit the concerned branch where you wish your child to be registered, as per the
Registration Schedule above
5. You will get a printed copy of the Registration Form at the branch against the payment of
a registration fee of Rs. 500/- (please collect the receipt).
