JUDICIAL AFFIDAVIT
I, PANCHITO SERAPIO, of legal age, Filipino, married, and a resident
of Barangay Cavite East, Palo, Leyte, and after having been duly sworn to in
accordance with law, hereby deposes and states.
PREFATORY STATEMENT
That the taking of my Judicial Affidavit was administered and
supervised by PMSg Mon D Dumlao and was done at Palo Municipal Police
Station, Palo, Leyte, and I am fully aware and cognizant of the questions
asked of me by PMSg Dumlao and my answers thereto are under oath being
aware that I may face criminal liability for false testimony or perjury. I
likewise requested that the questions asked of me be in English as well as
my answers thereto also in English which I speak and understand for clarity
and that the other information and/or requirements Sec. 3, A.M.12-8-8-SC
as explained to me by PMSg Dumlao are contained in my Judicial Affidavit
the rest which reads, to wit:
Q1: Please state your name and other personal circumstances?
A1: I am Panchito Serapio, of legal age, married, government
employee, and a resident of Acebedo Avenue, Barangay
Cavite East, Palo, Leyte.
Q2: Mr. Witness, where were you on June 29, 2021, at around
10:00 o’clock in the morning?
A2: I was standing across our house waiting for a passenger
tricycle in Acebedo Avenue, Barangay Cavite East, Palo,
Leyte.
Q3: Did you notice any untoward incident that happened while
you were standing along that street.
A3: I saw three (3) persons scaled the fence of M. Luiller
Pawnshop.
Q4: What happened after they scaled the fence of M. Luiller
Pawnshop?
A4: They entered the premises of the pawnshop by breaking
the ceiling thereof.
Q5: Where is that M. Luiller Pawnshop located?
A5: It is in Acebedo Avenue, Barangay Cavite East, Palo,
Leyte.
Q6: What happened next, if any?
A6: After a few minutes, I saw the three (3) persons went out
the premises of M. Luiller Pawnshop. They were
holding bag of undetermined contents.
Q7: Did you know these three (3) persons who barged into the
premises of M. Luiller Pawnshop and went out with bags of
undetermined contents.
A7: Yes, sir.
Q8: How did you know them?
A8: Because they are also residents of Barangay Cavite East,
Palo, Leyte.
Q9: Can you state the names of these three (3) persons?
A9: Yes, sir.
Q9: What are their names?
A9: Paquito Diaz, Romy Diaz, and Zosimo Diaz.
Q10: How did you know their names?
A10: Because they were also residents of Barangay Cavite East,
Palo, Leyte, the place where I am residing.
Q11: Did you report the matter to the police authorities?
A11: At first I did not report the matter to the police authorities
because of fear.
Q13: Is there anything else you may add to your testimony?
A13: That is all for the meantime, sir.
