Dear Respondents,

Good Day!

We, the researchers of Grade 12 I- Jeruel, are currently conducting a study


entitled “A Comparative Analysis Between Morning Shift and Afternoon
Shift and the Significant Effect on the Academic Performance of Selected
Grade 12 Students of Different Strands in Philippine Christian University-
Dasmarinas Campus S.Y. 2018-2019” in order to fulfill our requirement in our
Research Subject.

In relation to this, we are humbly asking for your time and participation to fill out
this survey questionnaire as honest and fair as you can. We are expecting your
unbiased and dependable answers for it will greatly affect the results of our
research study. Moreover, we assure you that your personal details will remain
confidential. Also, the data that will be gathered will be used for academic
