INSTRUCTIONS:
(Please read the instructions carefully before submission of this application)
1. The age of the candidate must be between 12 and 14 years on 1st April of the year of
Admission.
2. At the time of submission of application form, the candidate must be studying in 7th or 8th class.
3. The application form and the photographs must be attested by the Principal/ Headmaster of the
School in which the candidate is studying.
4. Domicile Certificate of the candidate duly attested by gazetted officer must be attached along-
with the application.
5. Legal Guardian of the candidate can only sign/countersign the application form in case the
father of candidate is dead. In case the father of candidate is serving abroad, he will send a certificate
duly signed by the Gazetted Officer of Department in which he is serving with the application form..
6. FC wards to apply through HQ FC, KPK, Peshawar as per their policy letter No. 1401/36/x/Edn
dated 18 June 2003 (Open merit amongst the FC Wards).
7. Candidates belonging to Razmak sub Division will countersign their forms from APA Razmak
Sub Division.
8. Last Date: Last date for submission of application forms with registration fee of Rs. 2200/- is
10 November 2018. However, admission forms will be accepted with late fee of Rs. 600/- up to 20
November 2018.
9. Examination Schedule: Entrance Examination will be held on Sunday 16 December 2018.
Exam will be on the line of NTS Covering (English, Mathematics, Urdu and Islamiat) from 7 th Class
syllabus of KPK / FG Textbook Board.
Note: OVERAGE/UNDERAGE AND INCOMPLETE FORMS WILL NOT BE ENTERTAINED (FEE OF
UNACCEPTED FORMS WILL NOT BE RETURNED.
