Who may apply for reclassification?

Steps in Processing the ERF:
Identify the position for which an Equivalents Records Form is submitted. Determine the
classification level to which it belongs.
Note the credits claimed for:
1. Educational Attainment
2. Length of service rendered in
public/ private schools
3. Professional activities/Training
Only properly supported claims are to be credited
What is an Equivalents Records Form (ERF)
It is a document indicating the educational preparation, training, teaching experience, and extra-
curricular activities for professional growth undertaken by a teacher.
It is used to validate the classification level of teachers.
It is necessary that this document is evaluated properly as it is the determinant factor in the
allocation of the teacher’s salary range.
Required Supporting Documents
How to Compute Credit Units for Teaching Experience
1. Educational Attainment – supported by original transcript of records, special order/ certificate of
graduation or diploma. In case of partial units earned, an original certification signed by the registrar
should be submitted. For those claiming M.A. Equivalent, a certification from the school concerned as
to the number of units required for the degree program the teacher is pursuing should likewise be
submitted.
This must be supported by a curriculum/ prospectus for the degree/program
2. Length of Teaching Experience – original copy of the teacher’s service record duly certified by the
Administrative Officer of the division/ or Original Certificate of Employment from Private School
3. Professional Activities – original certificate of completion of training or certificate of attendance
signed by the director of training or seminar and source of authority for holding the activity.
How to Compute Credit Units for Teaching Experience
Teaching experience shall be based on the teacher’s Service Records. Only teaching service
with pay shall be credited. Leave of absence without pay shall not be considered for paid service in
public schools.
1 unit = for every 3 years service in public school
1 unit = for every 5 years service in private school
How to Compute Credit Units for Training
Only Professional Activities on the national, regional and division levels shall be considered for credits.
24 hours = 1 unit
A maximum of Five (5) units is allowed to be charged to Training/Professional Activities . Only
teachers with at least very satisfactory (VS)performance for the last 2 semesters should be
recommended for upgrading. The latest performance rating should be indicated on the ERF.
CRITERIA FOR RANKING MASTER TEACHER
DOCUMENTS TO BE SUBMITTED BY THE CANDIDATES:
A. Basic Requirements
1. Service Records (prepared by the Records Section and signed by the OA)
2. Latest appointment
3. Performance Rating for the last 2 years(prepared by the service records)
4. Class/Intermediate Program for the last 2 years
5. Certification under oath of the school head that the candidate is actually teaching
6. Transcript of Records with SO or RN
7. PRC license or certification of good standing issued by the said agency
B. Leadership Potentials and Accomplishment:
All claims for all criteria should be supported by a certification under oath signed by the school
head/supervisor and corroborated by 5 other personnel involved in the activity.
Criterion A – Introduced any of the following which has been adopted or used by the
school/district/division
3. Income Generating Projects
1. Approved proposal and write up signed by the ASDS
2. Certification/other documents showing where income generated was used/Financial statement
signed by the principal
3. Certification of the school head/teachers who adopted the materials
4. Certification for being the proponent of an income generating project for 2 consecutive years,
corroborated by 5 teachers and duly notarized.
Criterion B- Served as subject coordinator or Grade Chairman for at least 2 years
2. Adviser of Interests Clubs
1. Designation signed by SH or PSDS
2. CLASS/Intermediate program with teaching load on the SY being claimed
3. Action Plan approved by SH/PSDS
4. Accomplishment Report attested by the principal
Criterion C- Served as chairman of a special committee
1. Credited for curriculum/instructional materials are: modules, guide, manuals, lesson plan,
worksheet, exercises, workbooks
Documents:
1. Memorandum on the organization of the special committee signed by the SH/PSDS
2. Certification for serving as chairman corroborated by 5 teachers and duly notarized
3. Project proposal signed by SH/PSDS
4. Action Plan signed by the SH/PSDS
5. Copy of the outputs curriculum/instructional materials or prepared program
6. Write up of accomplishment signed by SH/PSDS
Criterion D- Initiated or headed or participated in an educational research
1. Letter request to the ASDS to conduct an action research
2. Approved research proposal and write up/research report signed by ASDS
Criterion E- Coordinator of community project or activity or of a program
Of another agency or coordinator of rural service improvement activity
(feeding, nutrition, agro-industrial fair for at least 2 years)
1. Designation signed by SH/PSDS
2. Certification as coordinator of the project for at least 2 years signed by the barangay
chairman/officials
3. Action Plan approved by SH/other agencies
4. District/School Memo/Programs where candidates is named as coordinator
5. Report of accomplishment attested by SH or PSDS same s/y with the action plan
6. Sample evidences of accomplishment as indicated in the action plan and report of
accomplishment for the s/y upon which the candidate is claiming

Criterion F- Organized or managed an in service training activity or other
similar activity at least in the school level (coordinator who organized or
managed INSET shall not be given points under this criterion, it is an inherent function as a
coordinatorship
1. Memo where the candidate is designated as over all chairman/organizer/manager of the INSET
2. Certification for organizing/managing the INSET, corroborated by 5 teachers and duly notarized
3. INSET Proposal with training design approved by the PSDS, If district level or school head for
school level
4. INSET report of accomplishment signed by the candidate and School Head for school level and
PSDS for district
5. Certification of Merit/Commendation
Criterion G- Credited with meritorious achievement such as 1) trainer for Contestant who
received prizes in the National, Regional or Division level Or 2) athletic coach of athletes who
received prizes (this must be either a DepEd sponsored or endorsed activity)
1. Memorandum cited as winning coach
2. Certificate of award as winning coach
Criterion H- authorship
Criterion I – Demo teaching
1. Approved lesson plan signed by PSDS/ES
2. MEMO/TIP/PROGRAM of activities where the candidate is cited as demo teacher
3. Certification stating the exact date when the candidate served as demo teacher and
corroborated by at least 5 teachers and duly notarized
THE REVISED GUIDELINES ON SELECTION, PROMOTION AND DESIGNATION OF HEAD
TEACHERS/SCHOOL HEADS
BASIC POLICIES
Applicants to Principal I position must pass a qualifying test. He/she must have an experience of
at least five (5) years in the aggregate as Head Teacher, Teacher-In-Charge, Master Teacher and
Teacher Ill.
The test shall evaluate the applicants in terms of the three (3) leadership dimensions to measure
executive and managerial competence. It may consist of paper and pencil, simulation and other modes.
The test shall be developed by the National Educator's Academy of the Philippines (NEAP) in
coordination with the National Education Testing and Research Center (NETRC).
The Regional Office shall simultaneously administer the test once a year in designated venues.
The appointment of a school principal shall be non-station specific.
Any vacancy for Principal position shall be open to all qualified candidate from within and
outside the division where the vacancies exist.
The Schools Division Superintendent shall designate Teachers-In-Charge in schools without
Principal items.
To become a Teacher-In-Charge, one must have at least three (3) years teaching experience
and undergo a screening process to be conducted by the Division Office.
Assignment of Head Teachers shall be one per subject area with priority on the core subject
areas, namely: English, Math, Science, Filipino and Araling Panlipunan.
COMPUTATION OF POINTS
Specific points are assigned for each criterion in the ranking for Head Teacher/Principal positions, as
follows:

A. Performance Rating (30 points)
The performance rating of the appointee for the last three (3) rating periods prior to screening
should be at least Very Satisfactory. The average of the numerical ratings shall be given points as
follows:

B. Experience (10 points)
Experience must be relevant to the duties and functions of the position to be filled, with every
year given a point but not to exceed ten (10) points. Every month of service in excess of one year shall
be given corresponding point.
Example: 1 yr. & 5 mos. 1 5/12 = 1.4 points
5 yrs. & 11 mos. 511/12 = 5.9 points

E. Potential (5 points)
This refers to the capacity and ability of a candidate to assume the duties of the position to be filled up
and those of higher level.
The Personnel Selection Board/Committee (PSB/C) shall determine the potentials of the candidate
based on each of the following components:
1. Communication Skills - 1 pt. Speaks and writes effectively in Filipino and English.
2. Ability to Present Ideas - 1 pt. Presents well-organized and precise ideas with marked command of
the language used.
