Doctora’s Compound, Brgy. Mayapa, Calamba City, Laguna and SPS. GERALD LAIDA
LACATAN & JENNIFER N. LACATAN both Filipinos, of legal age, with residence
address at Block 13, Lot 1, Madrid St., Ph2 Calamba Hills Village, Brgy. Barandal,
Calamba City, Laguna, after having been swon in accordance with law depose and
state:
THAT, we executed a DEED OF ABSOLUTE SALE dated 25 September 2014
before ATTY. PURSINO C. ORUGA docketed as Doc. No. 282; Page No. 57; Book No.
609; Series of 2014, wherein SPS. MARIA ADELINA S. BEJASA & RICO ALAN B.
BEJASA are the VENDORS of TCT No. T-060-2013017359 who sold to SPS. GERALD
LAIDA LACATAN & JENNIFER N. LACATAN as VENDEES;
THAT, there was a clerical error or mistake committed on our deed of absolute
sale, wherein it was mistakenly written that SPS. MARIA ADELINA S. BEJASA &
RICO ALAN B. BEJASA where “Principals/Grantors” instead of the proper designation
as “VENDORS” and that SPS. GERALD LAIDA LACATAN & JENNIFER N. LACATAN
were presented as mere “Atty-in-fact” instead of the proper designation as “VENDEES;”
THAT, in order to correct the said DEED OF ABSOULTE SALE the words
“Principal/Grantor” should be changed and properly understood as “VENDOR” and
that the words “Atty.-in-fact” should be changed and properly understood as
“VENDEE” in order to correct the designation, authority, rights and obligations of the
parties therein;
THAT this affidavit is being executed to attest to the truthfulness of the
above facts, to correct the DISCREPANCIES/ERRORS NOTED and for any lawful
purpose it may serve.

– 1/6 SHARE.”;
THAT to correct the DISCREPANCY, “share of LEONARDO S. STA ROMANA,

inside my Envelope which I misplaced inside our house and best efforts

THAT, to correct it, the Civil Status should be included in the title, it should
be, viz:

single; EDWIN CLEMENTE C. ESPIRITU, Filipino of legal age, single; CYNTHIA
