MESSAGE
The LUCENA CITY NATIONAL HIGH SCHOOL – SILANGANG MAYAO
EXTENSION (LCNHS-SMEx) is committed to creating an environment where all
learners can achieve success. The LCNHS-SMEx Planning Team analyzed school-wide
assessment data and identified our school accomplishments as well as our school
concerns. To improve our systems and structures, LCNHS-SMEx has adopted
strategies and steps for implementation. This plan will be shared at Faculty Meetings
throughout this coming school year 2022-2023. All the teaching and non-teaching
workforce is committed to this plan and will work together to ensure all of our learners
are in a safe and respectful environment that will foster individual learner success.
The content of the School Report Card (SRC) is the basis for advocating the
school’s situation, context, and performance to internal and external stakeholders. As
with the Local Government Unit (LGU), Non-Government Organizations (NGOs),
Religious Organizations, Business Institutions, and all the members of the community,
working together, the school’s vision and mission will be fulfilled through
collaborative effort.

SCHOOL PLANNING TEAM


The planning team has identified the persistent problems affecting the
academic performance of our students as reflected in the previous Mean Percentage
Scores (MPS) results. To increase the mentioned achievement, the team must formulate
measures or tools to address the concerns. The positive outcome of this endeavor is
expected not only the sole effort from the students and teachers, but with the
cooperative participation of the parents.
Likewise, there is a need to improve the quality of instructions among our
teachers to be more efficient and effective. Therefore, it is crucial that the teachers
should enhance their competencies by attending trainings, workshops, or seminars.
Furthermore, it is advised that the mentors should continue to hone their skills by
taking up further studies in their master’s and doctorate degrees.
The physical aspect of improvement of the school is in its urgent need to
maintain its functionality. Identified facilities such as laboratory, canteen, library,
clinic, etc. will be continuously monitored and repaired as the needs arise. The stability
of these facilities will depend on the proper utilization of the allocated budget of
different projects and programs that will be implemented.
The planning team, stakeholders, and all the persons involved are committed to
render their time and capabilities to create the enhanced school improvement plan. This

is to make it more comprehensive and attainable in line with the Department of
Education’s vision and mission in general.

Assess

School Current Situation

Lucena City National High School Silangang Mayao Extension was


established in 2016 which initially started under the vicinity of Silangang
Mayao Elementary School located at Purok Sampaguita, Brgy Silangang
Mayao, Lucena City. Over the years, the enrolment was continuously
increasing. Due to the increase of students’ population of LCNHS Silangang
Mayao Extension, the proponent decided to push through to move on to the
new school site located at Purok Rosal Brgy Silangang Mayao Extension
though the building is not yet finish and not yet conducive for learning
environment.

The learners temporarily occupied the covered court with partition and
tent. There is no available canteen, no faculty room as well as school head
office. There are 2 temporary functional comfort rooms one for male and the
other for female, but the source of water is not evident. There is no lavatory
and other facilities such as Science lab, HE lab, ICT room and other
laboratories. There are only 141 total number of armchairs and still needed
215 additional chairs to cater the total number of students. There are only 2

available blackboards that’s why the teachers are only using improvised
whiteboard using white cartolina and plastic cover. The source of electricity
was supplied from construction firm with the permission of the contractor.

At present we have 20 fully ventilated classroom. Eight are intended


for the 8 sections from grade 7 to grade 10, one room serves as the office of
the school head, another room was intended for clinic and isolation area then
another room for learning resource center, the rest were not yet structured
for its purpose like Science lab, HE lab, ICT room, Conference room. Ten
functional toilets with lavatory and enough supply of water from the deep
well. Now, and internet connection were established. A hand washing facility
was constructed.

The school is located along the newly constructed Lucena Diversion


Talao Talao Port Road. It is one kilometer away from the old Manila Road and
takes 20 mins to reach the city proper. The location of the school is an
agricultural area and it’s leading to the biggest port of Quezon.

Lucena City National High School Silangang Mayao Extension is


classified as medium school based on the number of pupils and teachers.
There are 13 nationally funded teachers including the teacher in charge, 3
males and 9 females. For non-teaching personnel there is 1 nationally funded
administrative officer, three security guards and three utility which are locally
funded.

The enrolment of LCNHS Silangang Mayao Extension over the years


has significantly increased. But for the past two years due to the pandemic
the number of enrollees has been affected. For the School Year 2019-2020
the school has 111 grade 7 enrollees, 96 grade 8, 84 grade 9 and 79 grade
10 with a total of 370 enrollees. In the year 2020-2021 the school has
decreased the number of enrollees with a total enrollee of 360 from grade 7
to grade 10. And for the year 2021-2022, the school has increased to 376
total enrollees from grade 7 to grade 10.

During the school year 2019-2020 the school has a total of 6 drop out
from grade 7 to grade 10 which is 1.62%. In the succeeding school year
2020-2021, the number of dropouts has been reduced to 1 which is 0.27% of
the total number of populations. In the year 2021-2022, the school reached
the target of zero dropout rate.

For the last three years the learners’ MPS does not meet the national
standard of 75% specifically the ENSCIMA subjects.

Based on the Numeracy level test result conducted Last June 2018, 10
out of 40 students in each class are non-numerates and have low skills in
solving problems.

On the other hand, during the Pre - test of students literacy from
grades 7 – 9, The total number of frustrated readers is 69, 122 instructional, C
and 81 independent readers. Meanwhile, the result of post-test, were only 25 H
frustrated readers from 69, 122 instructional readers exceed to 125 and
independent reached 123 from 81 students. Based on the data gathered after

PHIL-IRRI, there are still 25 frustrated readers that needs to be cater and P

Based on the nutritional status of the students gathered last school

year 2018-2019. There are 79 total numbers of severely wasted, 38 from R
grade 7, 27 from Grade 8, and 14 from grade 9. In wasted there are 68 total
wasted 21 from grade 7, 31 from grade 8 and 16 from grade 9. Therefore
147 students need to prioritize. 3

In terms of instructional materials there are insufficient learning


materials in Science 7, ESP 7, Math 7 and Filipino 7. However, in subjects like
English, AP and MAPEH are almost complete, for the school year 2018-2019.
In addition, there is no learning material available in English 8 and Science 9.
Moreover, there are limited learning materials available in the subjects
English, Math, Science, and Araling Panlipunan in Grade 10.

Despite the challenges being faced by the school, the said school is not
left behind in terms of awards and recognition. The school received the
