II. TECHNICAL REPORT
A. RATIONALE:
A beautiful and attractive smile exudes confidence, indicates happiness and conveys
success. Dental and oral health is an essential part of general health. However, We provide a
comprehensive service in dental kit.
A toothbrush is a dental instrument used for cleaning teeth, ideally in conjunction with
toothpaste or mouthwash. The toothbrush consists of a plastic handle and nylon bristles
attached to the head of the brush. Toothbrush occupies a prominent and very important place
among the identified available today. Toothbrushes are getting more complex and sophisticated
every year.
For keeping good health, care of teeth is one of the essential step. The particle of food
that stick to our teeth give birth to fouling smell and many dreadful diseases, which destroy the
gums and result to tooth decay.
Children with ages between 4-12 are prone with dental problems such as toothache,
gums disease, cavities, oral cancer, dry mouth and bacteria on the tongue and many more it may
cause if we cannot avoid this dental problems. We have 859 enrollees for the School Year 2018-
2019. Hence, students of Sta. Cruz Central School are in need of Dental care. As we care for our
students we aimed that all students will free from these dental problems.
II. OBJECTIVES:
The objective of this proposals are the following:
1. To be able to prevent teeth and gum diseases.
2. To be able to learned proper hygiene.

ACCOMPLISHMENT REPORT
Sta. Cruz Central School students and as well as staff are very happy that Dr. Geraldo A. Gines, Dentist I
and PTA President donated a 510 pieces of small toothbrush for Kinder to Grade III students, 510 pieces
of big toothbrush for Grade IV to Grade VI students including teachers and staff with 32 pieces toothpaste
which is distributed to all classrooms.
