QUIT CLAIM, RELEASE, AND WAIVER

KNOW ALL MEN BY THESE PRESENTS:

I, Angelica Pascual Cruz, of legal age, and presently residing at 432 Rizal St. San Jose, Baliuag, Bulacan
here by manifest and declare that

1. In (enter joining date) November 23, 2022 Inspiro Relia, Inc., ("the Company") engaged me as
Customer Service Representative;

2. On December 11, 2022, I was separated from the Company due to; Personal Matters

3. That I have received fully the sum of Pesos: 801.57, representing full and final payment of all my
salaries, wages, separation or termination pay, allowances and all other emoluments in connection with
my past employment with the Company, and by virtue of which, I hereby forever release the Company,
its officers, directors, representatives, agents, assigns, subsidiaries, affiliates, successors, and its
affiliates' subsidiaries (including, but not limited to the Philippine Long Distance Telephone Company's
[PLDT] subsidiaries) from any claims and liabilities whatsoever and I hereby irrevocably waive, any and
all claims, causes of action and/or judgments I have or may have in my favor against them in any
tribunal, now or in the future, arising from my employment and cessation of such employment, with the
Company;

4. I have been paid in full all the amounts and other benefits monetary or otherwise due me from the
Company , its stockholders, officers, directors, agents, and/or its employees, whether such amounts are
due me by virtue of any law and / or Company practice, arising out of or in connection with my
employment with the Company.

5. In consideration of the foregoing payment, I hereby release, remise, waive and forever discharge the
Company, its stockholders, officers, directors, agents and/or its employees from any action, whether
civil, criminal, administrative or otherwise, or from any claim of any kind or character which I had or now
have against the Company on account of my voluntary resignation/cessation of my employment in the
Company.

6. I acknowledge that I have no cause of action whatsoever against the Company, its officers or
employees in respect of any matter incident to or arising out of my employment with said Company.

7. I shall not at any time or in any manner, either directly or indirectly, divulge, or disclose or
communicate to any person, firm, or corporation, or use to the prejudice of the Company, its
stockholders, directors, officers, and consultants any and all information affecting or relating to the
business of the Company, its manner of operations, its plans, or other data which was disclosed to or
acquired by me in the course of my professional or official relationship with the Company, except when
such disclosure or communication is required by law or order of a court of competent jurisdiction;

8. I agree not to be engaged as a principal, agent, employee, consultant, contractor, or in any other
capacity, in any business or undertaking in the same line of business as the Company or which may
compete in any manner with the Company, for a period of one (1) year from the cessation of my
employment with the Company.
9. I shall not canvass or solicit business from any person who was a client of the Company, unless that
person is already an existing client of my new employer prior to the commencement of employment
with the latter.

10. I agree not to employ or attempt to employee or engage in any discussion or negotiation or assist in
the employment of any person who is employed by the Company at the date of cessation of my
employment or employed thereafter for a period of one (1) year from the cessation of my employment
with the Company.

11. Finally, I declare that I have read this document entitled "Release, Waiver and Quitclaim" and fully
understood every word of it and its meaning since the same has been translated and explained to me
and I have affixed my signature hereunder voluntarily and freely with the full and complete knowledge
of the meaning and intent of this document and of my rights under existing laws.
