EXTRA-JUDICIAL SETTLEMENT OF ESTATE WITH WAIVER
AND RENUNCIATION OF RIGHTS
KNOW ALL MEN BY THESE PRESENTS:
That this Deed of Extrajudicial Settlement of Estate with Waiver and
Renunciation of Rights, entered into this ________ day of May 2019, in Pio V.
Corpus, Masbate, made and executed by and among legal and compulsory
heirs of the deceased Yolanda Raypan Mativo-Dollison, namely:
1. VICTORINO TABLANTE DOLLISON, JR., widower, of legal age,
Filipino citizen and a resident of Poblacion, Pio V. Corpus, Masbate;
2. VICTORINO MATIVO DOLLISON III, married to Ma. Concepcion B.
Longares-Dollison, of legal age, Filipino citizen and resident of Unit
6, Sunray’s Townhomes, Carmencita Village, San Pedro Ext., Talon
Tres, Las Piñas City;
3. SHIELA MAY MATIVO DOLLISON-AGLAY, married to Ariel Aglay,
of legal age, Filipino citizen and resident of Poblacion, Pio V. Corpus,
Masbate;

5. CLEMENTE MATIVO DOLLISON, married to _____full name of ate
yvette________, of legal age, Filipino citizen and resident of Poblacion,
Pio V. Corpus, Masbate;
6. VICTORINO MATIVO DOLLISON IV, married to _____full name of ate
telay________, of legal age, Filipino citizen and resident of Poblacion,
Pio V. Corpus, Masbate;
7. SHIELA JOY MATIVO DOLLISON-BARCELONA, married to
_____Barcelona, of legal age, Filipino citizen and resident of
Poblacion, Pio V. Corpus, Masbate;
WITNESSETH, that:
WHEREAS, YOLANDA MATIVO DOLLISON, died intestate on March ___,
2019 at Masbate City, Masbate (copy of her death certificate is hereto attached
as Annex “A”) without Will or testament and without any debt in favor of any
person or entity;
WHEREAS, her only surviving legal and compulsory heirs are the herein
parties, namely:
VICTORINO T. DOLLISON, JR. – husband
VICTORINO M. DOLLISON III - son
SHIELA MAY M. DOLLISON-AGLAY – daughter
CLEMENTE M. DOLLISON – son
VICTORINO M. DOLLISON IV – son
SHIELA JOY M. DOLLISON-BARCELONA– daughter
WHEREAS, the deceased left a bank account with Development Bank of
the Philippines (DBP), Masbate City with Savings Account No. 0625-
540528-530;
WHEREAS, in view of the above facts and circumstances, the
abovementioned legal heirs are therefore co-owners of the

value/proceeds/money in the abovementioned bank account by virtue of the
law on succession.
WAIVER OF RIGHTS
WHEREAS, We, VICTORINO M. DOLLISON III, SHIELA MAY M. DOLLISON-
AGLAY, CLEMENTE M. DOLLISON, VICTORINO M. DOLLISON IV and SHIELA
JOY M. DOLLISON-BARCELONA (whose personal circumstances
are mentioned above) have agreed and covenanted to CEDE, TRANSFER and
CONVEY our respective shares/inheritance in the proceeds of the
abovementioned DBP Savings Account No. 0625-540528-530 unto our co-
heir/father VICTORINO TABLANTE DOLLISON, JR. (whose personal
circumstances are mentioned above);
NOW, THEREFORE, for and in consideration of the foregoing premises,
we waived and renounced, as by these presents, we hereby WAIVE and
RENOUNCE our respective shares, rights and participation, as stated above,
over the above-mentioned DBP Savings Account No. 0625-540528-530 in
favor of our father Victorino T. Dollison, Jr..
