IV Medication check-list
Introduction:-
It is the administration of medication through the vein. The drugs which
can't be absorbed from gastrointestinal tract or are irritating to the
muscles are being administered through the intravenous (IV) route. This
route of drug administration is less traumatic than intramuscular and
subcutaneous drug administration if a peripheral line is well
established, whereas the risk of infection is high. To prevent infection
the strict aseptic techniques are required to be followed.

Possible sign and symptoms

- Phlebitis- refers to the swelling, redness, heat tenderness and pain


related to local inflammation of the vein.

-Infiltration: Oozing of fluid into the tissue surrounding the vein


puncture site.

Signs/Symptoms:- Tightness, edema, discomfort, no back flow of blood


blanching of affected area and coldness on palpation.

If such symptoms are present, medication should not be administered


and new IV line must be initiated.

Steps of procedure check-list
Pre-procedural steps.

8.) Check the NAME of the medication,


prescription DATE and SIGN of the
physician along with EXPIRY DATE of
drug.
9.) Check for any discoloration/ any
precipitates in the medicine
10.) Do AST (Antibiotic sensitivity test) for
antibiotic medicines which are given for
the first time.
11.) Do proper drug calculation before
administration

3.) Use hand rub and after that do proper
gloving.
4.) Clean the top of vials with spirit swab
before loading, let it dry.
5.) Dissolve and load medicine according to
the calculation.
6.) Keep the filled syringe in the sterile pack.
7.) Check the cannula site for patency,
infiltration.
8.) Again explain before administration.
9.) Clean all the ports with spirit swab.
10.) Use green sheet under cannula site.
11.) Flush 2ml of normal saline.
12.) Add the loaded medicine into the pedia
set and add rest normal saline to make it
up to 30 ml.
13.) Connect the IV line with the cannula port
properly after cleaning it with spirit swab
14.) Set the flow on the recommended rate
by the physician.
15.) Do proper labeling of the medicine, date
& time on the pedia set.
16.) Stop the medication if the child becomes
lethargic/ hyperactive/ shivering or any
other local adverse reaction.
