AFFIDAVIT OF SINGLENESS

I, MARK JAYSON ABUNIN, of legal age, single, Filipino and


presently residing at 525th Engr. Village, Brgy. Camp Tinio, Cabanatuan
City, Nueva Ecija, after having been sworn to in accordance with law
hereby deposes and states: That

1. I was born on December 10, 2017 at Bamban, Tarlac to Lerma


Abunin;

2. I am applying as member of the Armed Forces of the


Philippines;

3. That one of its requirements is the submission of the affidavit


of singleness;

4. That, at present I am still single, not married nor have not


contracted any marriage into anybody elsewhere herein in the Philippines
and has no child;

5. That I executed this affidavit to affirm the truthfulness of the


above statements for whatever legal intent and purpose this may serve;

I, MARK JAYSON ABUNIN, of legal age, single, Filipino and
