Dear Mr.

Bradshaw,

I'm writing this to let you know that I need to take sick leave because of severe migraines. Per my
physician's orders, I will need to remain off work until December 20.

I am including a letter from my doctor confirming his order that I take this time to rest and recover
my health. I sincerely apologize for any inconvenience my absence will cause.

I anticipate being about to periodically check my email and do some work from home, particularly on
the new building project on 7th Avenue. When I am back in the office on December 20, I will do all I
can to catch up on anything that I may have fallen behind on.

Please let me know if I need to complete any additional forms related to my sick leave or if you need
additional verification from my doctor. If I anticipate needing any additional time, I will let you know
as soon as possible. Please let me know if you have any questions or need clarification about
anything related to my leave or work. Thank you in advance for your understanding in this matter.

Sincerely,

Hi [Name of your manager or supervisor],

I’m sending you this email to inform you that I can’t make it to work today, [date], because of
[illness]. If something urgent comes up, I’ll be able to answer emails, but feel free to contact [name of

Thank you for understanding,

Dear Mr./Mrs. {Recipient’s Name},

I am down with fever and flu because of which I will not be able to come to the office for at least {X
days}. As per my family doctor, it is best that I take rest and recover properly before resuming work.

I have asked {colleague’s name} to check on my clients and will try to periodically check my email if
you need anything urgent.

Please grant me leave for the aforementioned period. If you need additional information, please let
me know.

Yours Sincerely,
{Your Name}
Dear Mr./Mrs. {Recipient’s Name},

I would not be able to join the office today because I am under the weather and have symptoms of
viral infection. I went to the doctor yesterday and he prescribed {X days} of rest.

I asked {collegue name} to handle my pending tasks, while I am away. I will be available via email for
any urgent needs.

I have also attached the doctor’s note to this email.

Thank you for understanding.

Dear [name of the supervisor],

I'm writing to inform you that I need to take sick leave from October 12 through 25  because of severe

back pain. My physician recommended that I take this time off to rest and recover. I have enclosed the

doctor's note with this letter. I apologize for the inconveniences that my absence will cause and look

forward to returning to the office upon recovery. I will complete any pending projects from home

whenever possible and do my best to catch up when I get back to work.

Please inform me if there is any other additional document that you may require in regards to my sick

leave.

Thank you for your consideration in this matter.

Unfortunately, I need to take my sick leave starting on [Date] for [Number of days].

[Insert formal summary of the sick leave, whether you'll be eligible for FMLA or Paid Sick Leave.]

I have been in touch with the HR department to determine the next steps I'd need to take as well as
what's available in regards to my PTO, sick leave, and any FMLA benefits. They have the
requisite medical certificate and sick leave letter as mentioned in the employee handbook.
I’m available to discuss my medical leave and what it means for my position over the next weeks if
you'd like to connect.
