OVERVIEW:
This concept explores the nursing process as it applies to community/public health nurses working with
families, the “core unit of service,” in the promotion of health and wellness, illness prevention, and
overall improved health of the population. The delivery of care to families occurs in various community
settings, such as their homes, their work settings, clinics and outpatient departments. Although caring
for the family, as a unit of service, is an effective way to treat the population in the communities, a gap
often exists between family nursing theory, development, and practice.
LEARNING OUTCOMES:
• To recognize the components of the nursing process as they apply to enhancing family health.
• To adapt the typology of nursing problems in family nursing practice.
• To formulate family nursing care plans based on family health problems.
• To identify and illustrate the scale for ranking family health conditions and problems according to
priorities.

SCALE FOR RANKING FAMILY HEALTH CONDITIONS AND PROBLEMS ACCORDING TO
PRIORITIES
Criteria Score Weight
• Nature of the condition or problem presented 1
Scale: ♠ Wellness State 3
♠ Health Deficit 3
♠ Health Threat 2
♠ Forseeable Crisis 1
• Modifiability of the condition or problem 2
Scale: ♠ Easily Modifiable 2
♠ Partially Modifiable 1
♠ Not Modifiable 0
• Preventive potential 1
Scale: ♠ High 3
♠ Moderate 2
♠ Low 1
• Salience 1
Scale: ♠ A condition or problem, needing immediate attention 2
♠ A condition or problem not needing immediate attention 1
♠ Not perceived as a problem or condition needing change 0
♠ Computations help systematize priority setting by determining specific scores for each problem in the list.
Scoring:
• Decide on a score for each of the criteria.
• Divide the score by the highest possible score and multiply by the weight:
Score_______ x Weight
Highest Score
• Sum up the scores for all the criteria. The highest score is 5, equivalent to the total weight.
