1 The safety and abandon ship drills conducted may not be efffectively conducted.
1.1 The ship's emergency muster list posted was not updated with the current crew list on board.
1.2 The crew was not familiar with their assigned roles and duties during emergency. No one know who is the
assigned personnel donning the fireman outfit.
1.3 The crew was observed having difficulty in donning the fireman outfit. Donning of the fireman outfit with Corrected
one crew assisting, took 5mins and 45 seconds.

The maintenance of the ship's Life Saving Appliances and Fire Fighting equipment was inadequate.
2.1 The maintenance tags tied on the life-saving equipment and fire fighting equipment was not updated since
end January 2023.
2.2 One fire and foam hydrant on deck inculding one fire line ioslation valve was found seized and the need to
use the 'F' key to open it.
2.3 Two fire hose discharge nozzle was found broken.
2.4 Few fire nozzles sigthed in rusty condition and seized due to lack of maintenance and lubricant on the
nozzle.

Several fire hose discharge nozzle was inspected and found seized and rusty, lacking of maintenance.
2.6 The fire hose boxes was not properly closed and with broken securing latch.

Two (2) numbers of life buoys located at the forward mooring station was observed with cracked shell.
2.8 The lifebuoy with light was observed the light line tied to the life buoy life rope's ring and not to the life
buoy body.

The two life buoys (Port and Starboard) at the life raft deck was found having broken down rope and flaking.

The foam tank sounding level sighting glass was observed cloudy and not able to read the foam level.
2.12 The international shore connection was having its bolts/nuts in a rusty condition, dried and without any

The ship's crew was not provided with latest information of the current safety policies and certificates.
3.1
Old version copy of the SMM policies and DOC certifcate was posted in the common mess room.
4 The maintenanance of the ship's air vents on deck from the machinery spaces (engine room) was
inadequate.
4.1
The base pipe at the pentrating deck was observed to be heavily rusted and broken loose rust sighted.
4.2 The unused 'Oil boiler Discharge Line' was sheered off and uprooted from its base plate at the penetrating Repaired
deck.
4.3
The air vent flap located in the Foam room above the foam tank was observed detached and dangle.
4.4 The exhaust fan for toilet was unoperative - MLC Issue

Mushroom vent (Port / Stbd) on the rescue boat deck, serving the engine room was found seized.
4.6 One self-closing device for the pipe located on deck at the break of accomodation (Starboard) was found
seized in open position.
4.7 The cargo tank breather valve was found in seized position.
4.8 The bow thruster exhaust vent penetrating through the forward store CO2 room was having a short section
of the pipe heat lagging broken and torn.

One butterfly nut for forward store skylight cover was found with its wing nut broken.
5 The oil spill prevention is inadequate
5.1 The forward starboard deck scupper when in place were having gaps and not fully covered or provide liquid
tightness. (Located below the starboard shoulder).
5.2 The SOPEP Inventory list was not updated.
5.3 Both the cargo pumps was having minor leaks with rags and portable plastic containment tray placed
underneath the leaks.
6 The safety reminders for man entry into an enclosed space was inadequate.
6.1 The pre-man entry reminder for CO2 room was not posted.

The Deck Officer was not aware on the gas alarm activation for the cargo pump room
7 Mooring Safety
7.1 Both forward and aft mooring station was not painted with anti-skid paint.
8 Safe Access
8.1 The pilot ladder was observed having a 'D' Shackle tied to the eye. Corrected

The hard rubber wedge choke was not secured with a rope twine to prevent misalign or loose.
9 Navigation
9.1 No record of the performance test (PM) for the radar.
9.2 Chief Officer not familiar with the PM test.
9.3
Shipping circulars, PMC, PMN not available onbiard for year 2023 (Soft or hard copies).
10 Machinery
10.1 The local operating control of the main engine was not available
10.2 The sewage plant was reported having its based cemented due to leak.
11 Others and Housekeeping
11.2
The unused heating line on deck was having few section exposed pipe and with thick rust.
11.3 The paint store was noted with paint stack up and not adequately secured to prevent fall and spillage
during passage.
