As per verbal appointment dated 18th July, 2022 & acceptance the same for inspection and
stability check of hospital floors on a residential building situated at as address mentioned
above, we have carried out the same on 19th day of July 2022.
The Hospital is occupied on Partial Area on Ground & 1st floor, in a Gr. plus seven storey
RCC framed structure. Hospital providing full facilities within this structure managed in 3
OPD conulting plus 20 bed are includes General Ward for Male & Female, Deluxe suite,
Private room, ICU Ward, Dialysis ward, Major & Minor Operation Theater.
Keep in mind all facts of the building stability. We have drafted a detailed stability report to
viable the above recently repaired building.

EXCLUSSIONS:
Force Majeure shall mean any circumstances beyond the reasonable control of the consultant /
society, which prevent or impact the performance of the recently repair work, including, but
not limited to any of the matters listed below;
War Or Hostility
Riot or Civil commotion or any such national emergency
Earthquake, Flood, Tempest, Lightning, or any other such natural disaster.
Accident, Fire or Explosion on the site, not caused by the negligence of any members.
Law or Order of any Government or Government department, which impedes or
delays the work.
A mere shortage of materials, utilities or labour shall not constitute Force Majeure, unless
such circumstances are created due to any of the above Force Majeure.
If such a Force Majeure comes into operation and thereby delays or prohibits the society from
enforcing specific performance of this survey report, then the society shall give a written
intimation to the consultant, giving details of the circumstances constituting the Force
Majeure, provided such an intimation is given within 7 days from the operation of the Force
Majeure.
Finally, the overall responsibility of STRUCT-CARE CONSULTANTS is restricted to
technical advice. All the legal procedural matters and the execution at site will be the
responsibility of the user of this report, in case the same is not executed under our
supervision.

I, Vivek P. Sarvaiya, undersigned is the structural engineer registered with the Municipal
Corporation of MBMC for structural examination purpose having registration no: 17
I have carried out the structural examination of the aforesaid residential structure by distress
Mapping (Visual Survey). I do hereby certify that I have personally visited and checked the
entire structural component of the aforesaid structure.
Assessment of said partial hospital floor in a Gr. + 7 storey RCC structure which comprising
Commercial shop & offices on Gr. & 1st level combined to create structure which is
rearranged as per room requirement for hospital units also separate staircase & lift
arrangement within these floors
Based on our inspection of overall building condition & partial area of part of Gr. & 1st floor
is occupied for hospital unit which is examined internally and some information received
from the key owner during the course of the inspection of this structure, we are in opinion that
the structural quality of the said building DOES APPEAR SATISFACTORY on basis of
Visual inspection & hence it is safe for human habitation.
But as age of building increases & due to saline atmospheric condition some stresses will
induce on structural members likely to affect beam, column as well ceiling slab which need
periodic attention. The society & key owners are advised to keep periodic repairs and
restoration work as & when needed to add strength to the building which will head the
building in healthy condition. Therefore we are recommend to carry out periodic
maintenance/ repair work such as structural repairs by hand packed polymer modified mortar
& Leakage/ seepage sources on time to time.
On the basis of the examination and verification of the building and as required under
provision of section 265A of the Bombay Municipal Corporation Act, 1949, I submit the
following certificate.
I, Vivek P. Sarvaiya, hereby certify that the aforesaid building structure fit for human
habituation and Validity of this report is 1 years (One Year) subject to duly rectification of the
repair work wherever required.
