ACCOMPLISHMENT REPORT
S.Y.2016-2017
A. Student Development
Actively participated in the organization of the Science Club.
Evaluated students’ performance through administration of Pre-test/Post-test, NAT,
RAT, Periodical test, recitation and other Science activities that enhance students’ skills
and potentials.
Actively participated and joined in different homeroom and school activities like Quiz
Bee, Poster Making, Essay Writing, School Pageants and viewing of subject related Films.
Attended different seminars, symposium and workshop which enhance the teaching and
learning capabilities of both teachers and students.
Developed the sense of responsibility of the student by giving them assigned area in the
Science Garden and classroom/surroundings.
Encouraged students to participate in different school activities and contest which will
develop their knowledge and skills.
Enhanced the spirit of camaraderie and sportsmanship among students’ thru different
games in class, collaborative learning activities and group performances.
B. Professional Development
Co-assisted and co-acted as adviser of the Science Club/YES-O.
Acquired necessary books, materials and guides in Science 8 which is used in the
preparation of attainable daily lesson log and visual aids.
Attended Faculty and Conferences regularly.
Prepared teaching aids and instructional materials for successful teaching and learning
process.
Improved teaching skills and techniques to achieve desired results and outputs.
Ensured cordial relation with superiors, colleagues, parents as well as to students.
Give untiring support in all undertakings and activities of the school.
Extended immeasurable support and help in the success of activities and competition
joined by the school.
Participated in various seminars/workshops and in-service trainings:
Division Seminar/Workshop for JHS Teachers to Teach SHS at
Nueva Ecija HS.
Orientation on LIS, EBEIS and NSBI at Hotel Consuelo, Santa Rosa,
Nueva Ecija.
Inset seminar for Communication Skills Enhancement, SHS
Curriculum, Test Construction/Analysis, Subject Content, Lesson
Planning/DLL, ICT, Values Formation, Duties and Responsibilities
of Teachers and Administrators.
Inset seminar for Lesson Planning, SIM preparation, Test
Construction and Analysis and Teaching Strategies.
Seminar on Child Protection Program
Designated as School Property Custodian/Supply Officer.
Designated as LIS Coordinator.
Attended and completed the competency requirements for Events Management
Services-National Certificate III sponsored by the TESDA.
Established rapport and camaraderie among teachers and students.
C. Curriculum Development
Submitted Lesson Plan, Class Records, Test Questionnaires with Table of Specifications
in Science 8 on or before the given schedule and deadline.
Accomplished all forms and reports of students’ performance.
Integrated values in teaching and performing Science skills and concepts.
Ensured the implementation of Basic Education Curriculum (BEC) and K to 12
Curriculum.
Acquired more teaching guides and other supplementary references for better teaching
skills.
Determined the proficiency level, Mean, MPS and SD regularly as needed.
D. Physical and Instructional
Development
Maintained and beautify the Science Garden.
Help enhanced and maintained the Science Laboratory Room and Science corner at the
LRMDS.
Improved the Science Garden by repairing/fixing its fence and gate, and
propagating/purchasing additional plants.
Get involved in 2016 Brigada Eskwela Campaign.
Maintained and improved the Grade 8-Rose Classroom.
Landscaped and beautify the plant boxes and covered walk of the Grade 8 Building.
Maintained and managed the Stock Room.
Involved the community by tapping the parents and private sector help in developing
our school for the benefit of our students.
E. Community Involvement and
Extension Services
Active participation in the different programs and activities of Local Government.
Communicated with parents of students who had problems through home visitations.
Supported and implemented the Barangay Officials’ Policies and Programs.
