Year 2023-2024

Teachers
1. In line with the preparation and accomplishment of school forms for the End of School
Year 2023-2024, this Office announce the specific guidelines on the conduct of School
Level Checking of Forms on May 15, 2024, 9:00 in the morning at the Office of the
Principal.
2. All policy guidelines stipulated in the Deped Order No. 11 s. 2018 titled Guidelines on
the Preparation and Checking of School Forms shall be strictly observed.
3. Attached are the following enclosures for guidance and information:
Enclosure 1: Matrix of Activity
Enclosure 2: Technical Working Group (TWG)
4. All expenses relative to this event shall be charge in the School Fund subject to the usual
accounting and auditing rules and regulations.
5. Immediate and wide dissemination of this Memorandum is earnestly desired
