INTRODUCTION

The implementation of internship or professional practice programs becomes


an opportunity for university students to acquire technical knowledge and skills as a
foundation for individual and social development of man, and also to prepare for
their entry into working life. From now on, we will call Professional Practice, all
practical training activities referred to as Industrial Internships, Educational Practice
and Rotating Internships in companies or organizations, through which the
technical knowledge and skills of students are made effective.

Professional Internships allow for the optimization of the different activities


carried out in each of the work areas of the different organizations. Likewise,
spaces for participation will be established for students so that they can translate
their knowledge and skills into work practices, in order to complement their
academic training cycle, and their individual and social development. In this sense,
professional internships play a very important role, allowing students to improve
their theoretical and practical knowledge, developing skills and abilities in the
acquisition of new knowledge that favors their training.

CHAPTER I NAME OF THE INSTITUTION

DR. GENERAL HOSPITAL LUIS RAZETTI

GEOGRAPHICAL LOCATION

It is located in the Autonomous Municipality of Barinas, specifically between


Cedeño Street and Cruz Paredes Alley, geographically it is located northeast of the
State, in the sector called La Carolina, between Cedeño Street, intersection with
Cumaná Avenue.

HISTORICAL REVIEW

In the early years of the 20th century, Venezuela's industrial boom emerged
and as a result of the exaggerated growth of the population and the emergence of
the labor force, it became necessary to create medical assistance centers in all
states.

In the State of Barinas, construction of a hospital began in 1922, when the


president of the State of Zamora, ISIDRO CORDERO, decided to acquire the
mansion of the Sanguinetti estate and under the direction of maestro OLIVARES,
the plans and projects for the necessary facilities were drawn up. But it was really
under the government of General ELEAZAR LOPEZ CONTRERAS, when the
population growth saw the need to create the first health center in the state, at that
time the President General JORDAN FALCON, organized a charity board, in order
to collect funds for the construction of the work.
Among the collaborators were famous doctors. Those who decided to name
it after the honorable Dr. LUIS RAZETTI; to baptize the Hospital of Barinas and
upon completion of the construction, the Spanish surgeon Dr. Vicente Gómez La
Torre remained as director of this institution.

The Type III Urban Hospital was created in 1936 during the government of
General Eleazar López Contreras and at that time Carlos Jadan Falcón; initially it
was based in the facilities where the Mayor's Office of the Autonomous Municipality
of Barinas currently operates.

By 1950, under the professional direction of Rubén A. Díaz placed the


Barinas hospital under the direct control of the Ministry of Health and Social
Assistance; and between 1961 and 1970, such progress had been made that the
first anatomopathologist in Barinas was hired in Germany, in the person of Doctor
EBERHARD, who, with his knowledge, taught the doctors of Barinas the cause of

the death of so many patients from tuberculosis.

Later, in 1971, under the government of Dr. Rafael Caldera, the need to
expand and diversify medical assistance coverage was observed due to the
quantitative increase in the population. That is how, in that same year, construction
began on a more modern building adapted to the needs and demands of the
population.

In 1972, the corresponding decree for the construction of the Hospital with a
larger infrastructure was published in the national press and that is how President
Rafael Caldera placed, to the general delight, the first stone for its construction. For
three (3) years, its concrete mass was raised as a message of hope and on
December 5, 1976, under the presidency of Mr. CARLOS ANDRES PEREZ, the
solemn inauguration took place on a bright morning.
This organization is made up of (1) a Director, (1) a Sub-Director, (1) an
Administrator, (61) sixty-one departments, each one of them with their respective
specific functions and a sufficiently trained staff to carry out the activities carried out
there and where the levels of command go in a straight line, that is, decisions are
made at higher levels and delegated to the other departments.

The first director of the new headquarters of the Dr. Luís Razetti Hospital
was Doctor Manuel Vicente Gelambi, and its first head nurse was Lourdes Rangel
de Gómez, who were responsible for the organization of this center, together with
the existing staff at that time.

Its name is due to the figure of Doctor Luis Razetti, a Caracas doctor with a
long professional career, who provided countless contributions and knowledge in
relation to medicine to the Venezuelan people, a source of ethical and professional
inspiration.

MISSION

Guarantee the right to health contemplated in the Constitution of the


Bolivarian Republic of Venezuela through timely, humanistic and quality
comprehensive care for users, using all available resources in order to contribute to
a healthier life.

VISION

To be a public organization providing health services oriented towards


efficiency, through the best use of resources; combining technology, science,
finance, physical and environmental aspects, with an effective human resource that
generates an organizational culture in which the values of responsibility,
commitment, unity and service combined with ongoing training, lead to
comprehensive care to improve the quality of life of the population.

OBJECTIVES OF THE ORGANIZATION

GENERAL OBJECTIVES:

V Provide timely, humanistic and quality comprehensive care to the users of the
Dr. Luis Razetti Hospital – Barinas, in order to contribute to the satisfaction
of their health needs.

SPECIFIC OBJECTIVES:

V Develop methodology for monitoring and evaluating processes, actions and


quality of care in services.
V Establish and promote guidelines to strengthen Epidemiological Surveillance
of Health problems.
V Develop criteria to ensure the provision and logistics system of supplies,
medical equipment and clinical support.

V Promote good relations between the community and the institution.
V Projector the Hospital to the community.
V Develop training and development programs for all staff on an ongoing and
consecutive basis.
V Increase and improve interpersonal relationships in the institution to achieve
high-quality management
V Improve accessibility, coverage and quality of health services.
V Provide timely, efficient care with universality, equity, quality, warmth,
confidentiality and privacy.
• Promote citizen participation in the health problems of the
population.
V Develop guidelines for the reorientation of targeted offers to respond to the
deficit of attention to specific health needs.
V Reduce morbidity and mortality from any disease and age group in the
population.
V Increase coverage of the different vaccines included in the vaccine-
preventable diseases program.
V Reduce gaps in existing health indicators (by gender, socioeconomic level,
geographic areas, ethnicities.
V Develop and promote health information, education and communication
strategies.

FUNCTIONS OF THE NURSING DEPARTMENT

• ADMINISTRATIVE FUNCTION
It involves the effective and efficient application of the management
process in daily practice to achieve the restoration of the patient's health.

• CARE FUNCTION
In this role, the nursing professional applies theoretical-scientific
knowledge in the use of nursing procedures and techniques that contribute to
offering direct care to restore health and enable the patient to integrate into
family and productive life as soon as possible.

• TEACHING FUNCTION
In this role, nursing staff plays the role of teacher, either formally or
informally, sharing knowledge about health education that leads to the
modification and/or learning of behaviors that are favorable to the health of the
individual or family.

• RESEARCH FUNCTION
In carrying out this function, the nursing professional has the obligation to
constantly deepen his or her knowledge, as does any branch of human
knowledge. Therefore, research is needed in order to increase the body of
knowledge, related to both theoretical principles and professional practice.

MISSION:

Its fundamental purpose is to provide nursing services based on the


sciences of human care, to offer users: hospitalized patients, family and

community comprehensive care under a holistic concept, emphasizing
prevention, assistance and health restoration strategies, supported by ethical
principles of respect, equity and responsibility.

VISION:

To be the leader in patient, family and community care based on the


nursing care and research process, distinguished by its spirit of responsibility
and service, achieving total customer and community satisfaction, within the
framework of quality and competitiveness.

PHILOSOPHY:

Provide quality nursing care to the user, without considering race,


religion, sex or socioeconomic level, projecting a high sense of responsibility
and human relations through a high academic level, also developing a triple
function: care, research and teaching, having as a clear objective to obtain
excellence, which will be reflected in the benefit to the patient and his family.

FUNCTIONAL OBJECTIVES

1. Provide comprehensive nursing services to patients, families and


communities in health promotion, prevention, recovery and rehabilitation.

2. Develop training courses for nursing staff in accordance with the


institutional human resources training policy.
3. Evaluate nursing care and programs within their competence and propose
necessary adjustments and changes.
4. Establish and maintain up-to-date technical standards and procedures for
the Activities under its jurisdiction.

CHAPTER II LEGAL BASIS

CONSTITUTION OF THE BOLIVARIAN REPUBLIC OF VENEZUELA

Article 83: Health is a fundamental social right, an obligation of the


state, which will guarantee it as part of the right to life. The State will promote
and develop policies aimed at improving the quality of life, collective well-
being and access to services. All persons have the right to health protection,
as well as the duty to actively participate in its promotion and defense, and to
comply with the health and sanitation measures established by law, in
accordance with the international treaties and conventions signed and ratified
by the Republic. According to this article, all communities have the right to
receive health care to reduce health problems.

Article 84: To guarantee the right to health, the State will create,
exercise leadership and manage a national public health system, of an
intersectoral, decentralized and participatory nature, integrated into the social
security system, governed by the principles of free access, universality,
comprehensiveness, equity, social integration and solidarity. The national
public health system will give priority to health promotion and disease
prevention, guaranteeing timely treatment and quality rehabilitation. Public
health goods and services are the property of the State and cannot be
privatized. The organized community has the right and duty to participate in
decision-making regarding the planning, execution and control of specific
policies in public health institutions.

Article 85: Financing of the national public health system is the


obligation of the State, which will include fiscal resources, mandatory social
security contributions and any other source of financing determined by law.
The State will guarantee a health budget that will enable it to meet the
objectives of its health policy. In coordination with universities and research
centres, a national policy for training professionals and technicians and a
national industry for producing health supplies will be promoted and
developed. The State will regulate public and private health institutions.

ORGANIC LAW OF HEALTH

Article 1: The purpose of this Law is to regulate the actions that make
effective the universal right to health enshrined in the Political Constitution of
the Republic and the law. It is governed by the principles of equity,
comprehensiveness, solidarity, universality, non-waiver, indivisibility,
participation, plurality, quality and efficiency; with a focus on rights,
interculturality, gender, generational and bioethics.

Article 2: All members of the National Health System, in carrying out
health-related activities, shall be subject to the provisions of this Law, its
regulations and the standards established by the national health authority.

Article 3: Health is a complete state of physical, mental and social


well-being and not merely the absence of disease or infirmity. It is an
inalienable, indivisible, non-waivable and intransigible human right, the
protection and guarantee of which is the primary responsibility of the State;
and the result of a collective process of interaction where the State, society,
family and individuals converge to build healthy environments, surroundings
and lifestyles.
CODE OF ETHICS

Article 2: Every nursing professional must act with respect for life, for
the total integrity of every human being, providing and ensuring the promotion
and maintenance of health. Based on the above, it can be established that the
action of nursing in the community is necessary; it is responsible for ensuring
the preservation of the health of the people in its care, while collaborating to
bring about changes in attitude that favor the care of each human being as a
person.

UNDERGRADUATE STUDIES REGULATIONS OF THE NATIONAL


EXPERIMENTAL POLYTECHNIC UNIVERSITY OF THE NATIONAL
ARMED FORCES

Article 17. Depending on the curriculum design of the course, it may


be offered during the day, at night, or both. Depending on the academic
degree to be achieved, the duration of the courses varies as follows:

a. Higher University Technician, whose minimum duration is four (4)


academic periods in the day shift and five (5) in the night shift,

b. Bachelor's degree or equivalent, which will have a minimum duration of
eight (8) academic periods in the day shift and nine (9) in the night shift.

c. Engineer, which will have a minimum duration of eight (8) academic


periods in the day shift and ten (10) in the night shift.

Article 18. The duration of the academic hour in the classroom will
correspond to forty-five (45) minutes of dedication.

Article 19. The study plan is understood as the ordered and structured
set of subjects, activities and academic experiences integrated by training
areas, in accordance with principles, guidelines, criteria and general
objectives established in the curricular proposal, which the student must take
to achieve the training purposes of a curricular design.

Article 20. Academic credit is the conventional value that will be used
to measure the student's academic workload, and will be explicitly established
in the curricular designs. Each subject will be expressed in credit units.

Article 26. Professional Internships are mandatory curricular units, a


graduation requirement within the curriculum of the courses that contemplate
them; they will be carried out in the organs and entities of the Bolivarian
National Armed Forces and public institutions as a priority, and in those
institutions that the student selects, with prior authorization from the core
council. Everything relating to its development will be established in the
internal regulations issued for this purpose by the University Council.

OBJECTIVE OF THE INTERNSHIPS.

GENERAL OBJECTIVE

To develop the knowledge, skills and abilities acquired during academic


training, to every patient who attends the General Hospital Type III "Dr. Luis
Razetti" of the Barinas municipality, Barinas state.

SPECIFIC OBJECTIVES.

V Comply with the different functions, activities and tasks of nursing within
the service.
V Identify the main health problems in the environment.
V Provide timely assistance to patients who require it.

SURGERY AND TRAUMATOLOGY AREA

It is made up of specialized personnel, dedicated to patient care,


providing timely, efficient and effective health care to users, with the purpose
of satisfying the needs and permanence of users who enter the surgery and
trauma unit on the 4th floor of this hospital institution.

To provide a high-quality service in terms of promotion, prevention and


rehabilitation, in order to reduce complications associated with hospital stays
and their integration into society by providing nursing care in a timely manner.

The surgery and trauma unit seeks to provide quality care for users,
their families and the community in general, with committed nursing staff
forming part of the multidisciplinary team for the application of the acquired
theoretical and scientific bases to solve the bio-psycho-social needs of the
person.

It deals with comprehensive adult health care, characterized by its


humanized, non-surgical clinical approach, which covers everything from
primary care to tertiary care.

MISSION

It allows a comprehensive approach to the adult patient in the hospital


setting. As a core of clinical specialties, it has two particularities: on the one
hand, it fulfills a primary function with the assessment/diagnosis and initial or
total management of a pathology, but on the other, it is a reference for other
specialties for the comprehensive management of the multi-pathological
patient.

VISION

It seeks to be the link between the different levels of healthcare to


guarantee adequate service to patients involved in the healthcare process.
We aim to achieve complete citizen satisfaction through our services, meeting
their expectations and those of the institution's professionals.

OBSTETRICS AREA

Week 5 and 7 (10-07-2019 to 10-12-2019) (10-21-2019 to 10-26-2019)

It is the operational Organic Unit in charge of programming, executing


and evaluating the actions and procedures for the comprehensive care of
women during the reproductive process. In this sense, it promotes, protects,
recovers and rehabilitates the health and well-being of the mother during
pregnancy, childbirth and postpartum, in both normal and abnormal
circumstances, with emphasis on complicated or high-risk pregnancies.

MISSION

Provide comprehensive women's health care in the Obstetrics Area


with activities for promotion, prevention, control of pregnancy, childbirth and
postpartum, with an emphasis on patients with high obstetric risk, with ethical,
highly trained, up-to-date professionals using cutting-edge technology.

VISION

To be recognized as a leading department in obstetric patient care for


its quality, warmth and its great problem-solving capacity with cutting-edge
technology and the latest advances in the specialty and with professional
ethics.

PEDIATRIC MEDICINE AREA

Week 6 (10-14-2019 to 10-19-2019)

This service is able to care for children in all stages of development up


to adolescence (14 years 11 months), providing pediatric emergencies and
outpatient consultations 24 hours a day, supported by an on-call team that
includes a Pediatrician and a Resident Pediatrician.

MISSION

The Pediatrics Department of the Dr. Luis Razetti Hospital provides


efficient and effective care in medical treatment pathologies together with the
multidisciplinary team, applying scientific knowledge, technical capacity and
human resources with high humanistic value.

VISION

To be the best service par excellence with specialized care from the
medical and nursing team for the Llanera region, with great warmth, quality,
teaching and research.

ACTIVITY PLAN ACHIEVEMENTS

During the development of the different activities at the General


Hospital Type III "Dr. Luis Razetti", thanks to the effort, they have been
achieved in a very satisfactory way, knowing that a difficulty is an opportunity
in disguise; through the development of the different nursing functions, the
improvement of establishing a nurse-patient relationship and good dynamics
with the different members of the health team is achieved. A notable
improvement is achieved in the skills and abilities in routine and/or main
activities of the nursing professional.

It should also be noted that the development of a clinical case allowed


the identification and satisfaction of patient needs and the resolution of health

problems; through an orderly description of both the events that occur to a
patient during the course of an illness, as well as the complementary data
provided by diagnostic procedures, the course of clinical reasoning, the
diagnostic conclusion, the treatment used and the patient's evolution. Using
tools that allow planning the process that includes data collection, nursing
diagnosis, establishing short- and long-term objectives and the most
appropriate nursing care to achieve them.

Likewise, by being in the different areas where the professional


practices were developed, small goals were achieved that contributed to
strengthening individual growth in the professional field, with the constant
training and advice of institutional tutors fulfilling a role as guides for the good
development of professional practices.
KNOWLEDGE ACQUIRED IN THE INTERNSHIPS

Through the development of internships, we were able to strengthen


and consolidate comprehensive professional training through learning
methods that allowed us to develop competencies in the knowledge of the
Nursing discipline in work contexts closely linked to our training, achieving the
integration of skills and attitudes acquired in the classroom and in clinical
fields, facilitating insertion into the workplace.

In other words, completing these internships and working in different


areas allowed me to gain in-depth knowledge of all the activities carried out to
improve my knowledge of the development of a nursing professional in the
hospital setting. In this way, it was possible to acquire some knowledge in
professional hospital practice, such as:

V Identification of health problems in their environment, as well as


proposing and influencing solutions that promote individual and family

well-being.
V Participation in actions to promote health and prevent diseases within
the scope of their performance, responding to the problems of their
context.
V Planning of the care actions required to resolve nursing diagnoses
identified through a comprehensive assessment of the individual.
V Implementation and evaluation of nursing care interventions aimed at
addressing human responses within the health-disease process at any
stage of the life cycle.

V Organization and coordination of the actions of the care of


nursing, which allowed it to evaluate and improve the quality of service
delivery.
V Use of nursing research methodology as a knowledge tool to describe
and explain phenomena related to health care in the human life cycle.

Therefore, the knowledge acquired in the different tasks to be


developed within the institution was able to lead to an improvement in skills
and abilities, which were expanded through the knowledge acquired at the
University during the previous semesters of study of the career, thus
reinforcing intellectual and work capacities in the field of analysis.

CHAPTER IV CONCLUSION

Professional internships are the means through which Nursing students


are helped to enhance and apply the different knowledge acquired (cognitive,
procedural, attitudinal, research and evaluative) in the real field of work and
where in the future they will develop as expert professionals in the
performance of their activities. Professional practice is the guided and advised
exercise, where the student participates in the solution of health problems that
affect the population, highlighting interventions in the promotion of health and
safety, prevention of risks and disease, treatment and rehabilitation of the
human being, family, community and/or groups.

For these reasons, professional practice allows students to acquire
significant learning experiences in the receiving unit and to strengthen the
cognitive, attitudinal, procedural, evaluative and research skills acquired
during their academic training; it also helps to increase the culture of Nursing
through personal improvement and professional updating.

From this perspective, it can be assured that the objectives proposed in


this final report of the hospital professional internships were fully met, since
they were addressed in the best possible way by implementing the necessary
knowledge for the successful outcome of the work.

• To the Teachers:

The National Experimental Polytechnic University of the Armed Forces


(UNEFA) encourages you to continue with greater enthusiasm in the
wonderful work of guiding, directing and teaching all student interns.

• To the Students:

UNEFA interns are asked to comply with all regulations requested by


teachers with respect, interest, and professional ethics. Likewise, nursing
interns are encouraged to love the profession by doing everything possible to
contribute to the health of patients and to continue preparing themselves
academically to further optimize their role in nursing.

Get involved in the different tasks of the department where you do your
internship, in order to gain comprehensive training, even if you don't have any
knowledge about them.

Do not limit yourself to the tasks assigned in the activity plan, but take
on additional assignments in search of professional growth.

• To the Nursing Staff:

To help students while they are presenting their internships, to provide


the acquired knowledge they have to students who do not keep that
knowledge.
