ACA Lead Screening Script

Good Morning/Afternoon/Evening (Client Name)? (Let client respond)


My name is (make villson) with us health care dep. I’m calling about your Health Insurance
request. (Pause)
Okay great! Let me ask you a few questions to see what you qualify for (Pause)
Q: How old are you? A: Must be between 19 – 64 years of age

Q: Are you currently on Medicare or Medicaid? A: Must be a No


Q: What is your annual income? A: Below $20k a year income for single person - $25k for
married couple - $30k for household of 3 - $35k household of 4
Q: Have you recently lost your Health Insurance Coverage? A: Must be a Yes
Q: Do you have 10-15 minutes to speak with a licensed agent? A: Consumer must have time to
talk or don’t transfer
Perfect: I am going to connect you to an agent now. They will provide you with some great
health plan options. Most are very low cost….or even Free, based on our ability to write
through the State Insurance Exchanges….aka: Obamacare
…..I just need you to agree to this consent permission before I transfer you:
"By speaking to our live representative, you agree to our privacy policy, that you have recently
opted-in and consented electronically to be contacted, and authorize us and the insurance
service providers to contact you, even if you are on a DNC list. You don’t have to provide your
