Participant's Membership Document (PMD)
GENERAL PROVISIONS
Preamble:
This is to acknowledge that the applicant (hereinafter called the Participant), having submitted the Application
Form along with the associated documents and undertaking to pay the Takaful Contribution, as more fully
described in the Participant's Specific Schedule (hereinafter referred to as the "PSS") attached hereto:
i. is accepted, as per the PSS as a member of the Group Family Participants Takaful Fund (hereinafter
referred to as the GFPTF) operated by EFU Life Assurance Ltd. (hereinafter referred to as the Window Takaful
Operator) under EFU Family Takaful Waqf Fund (hereinafter referred to as the Waqf Fund).
ii. Being a member of the GFPTF, the Participant is acknowledged as a beneficiary of the benefits declared
by the GFPTF from time to time under this PMD; in accordance with the Waqf Deed and the Participant
Takaful Fund (PTF) Policies. The PTF Rules are available at the Head Office of the Window Takaful
Operator.
iii. Subject to the Participant continuing as a member of the GFPTF and complying with its undertaking
and the declarations made in the Application Form, the Participant may be paid by the GFPTF as one of its
beneficiaries against the Takaful Benefits, in the manner and to the extent as stated hereunder.
iv. No payment in respect of any Contribution shall be deemed to be payment to the Window Takaful
Operator unless a printed form of receipt for the same, signed by an official of the Takaful Operator, shall have
been given to the Participant.
v. Notwithstanding anything above, cover under this PMD shall not commence until the Contribution, as
stated in the PSS hereof, has been paid or guaranteed to be paid in the manner as stated in the PSS or as
expressly agreed and stated therein.
Therefore, this PMD witnessed that this Membership shall at all times and under all circumstances be subject to the
Conditions and Stipulations printed hereon, which Conditions and Stipulations constitute the basis of this
Membership, and are to be considered as incorporated in and forming part of this PMD
EFU TAKAFUL FAMILY HEALTH PLAN
PROVISIONS AND CONDITIONS
1. DEFINITIONS
In these provisions:
ACCIDENT means accidental bodily injury which is caused solely by violent, external and accidental
means resulting directly and independently of all other causes.
BANK means Mobilink Microfinance Bank Ltd.
BENEFICIARY means a person or persons appointed by the Insured Member (under the provisions of the
Insurance Ordinance 2000) receive the benefits payable under the policy
CAESARIAN SECTION SURGERY means a surgical procedure in which incisions are made through
women’s abdomen and uterus to deliver her baby.
COMPANY means EFU Life Assurance Limited.
COMMENCEMENT DATE means the date on which the Contribution is paid after acceptance of the
application for group family takaful.
CONTRIBUTORY TAKAFUL means Takaful coverage for which the Participant contributes toward.
CONFINEMENT DATE means Date on which the Member is registered and interned as a bed-patient in
a Hospital and incurs a daily room charge, whether or not borne by the Member himself.
CLAIMS HELPLINE means the telephonic helpline on which the Member or any of his family members
accompanying him at the hospital must pre-advice about his hospitalization, any time before discharge
from the hospital, to the Company or an entity authorized by the Company.
DAILY BENEFIT means the benefit payable to the Insured Member for each day of Hospital confinement,
as specified in this Policy.
ELIGIBLE PERSON means an accountholder of the Bank who is eligible for takaful cover under this policy
in accordance with Clause 4 of these Terms and Conditions.
EFFECTIVE DATE & TIME OF THE POLICY means the date and time on which the cover becomes
effective which will be from 00:01 hours on the Commencement Date or the date following the date on
which the Takaful Contribution is paid after acceptance of the application for Takaful Cover.
GROUP FAMILY PARTICIPANTS TAKAFUL FUND means the collective fund hereinafter referred as
GFPTF under the Window Takaful Operator Waqf Fund into which all Group Family Takaful Contributions
for Takaful Death Benefits as well as Takaful Additional Benefits (if any) are pooled. The benefits of GFPTF
belong to the Participants with certain conditions for their mutual help and GFPTF is managed by the
Window Takaful Operator as Wakeel under the Islamic Concept of Wakalah.
ELIMINATION PERIOD means a period of fifteen (15) days from the effective date during which claims
arising other than Accident will not be payable.
FAMILY COVER means coverage of Member, his eligible spouse and eligible children (unmarried) under
this Policy.
FAMILY MEMBER means a spouse of Member between age of 18 and 65 years (nearest birthday) and
unmarried and economically dependent children of Covered Member between age of 6 months and 21
years (nearest birthday).
HOSPITAL means an institution that is:
• Properly licensed to provide medical care in accordance with the laws of Pakistan;
• Is primarily engaged in providing diagnostic and laboratory tests, medical evaluations, medical
treatment & services and surgical facilities;
• Has 24 hours-a-day nursing service by registered graduate nurses under the permanent
supervision of in-house Physicians in charge;
• Maintains proper in-patient facilities with documented protocols and procedures; and Maintains a
daily medical record for each of its patients, which is readily accessible to the Company.
INDIVIDUAL COVER means coverage of only Member under this Policy.
INJURY means bodily injury which is caused solely by violent, external and accidental means and
resulting directly and independently of all or any other causes after the Commencement Date.
MEMBER means an Eligible Person who is to be included in this policy after giving his valid consent and
paying the required takaful contribution amount.
INPATIENT means a Member OR Family Member confined in a Hospital as a registered bed patient for
at least a consecutive period of 24 hours.
POLICY YEAR means the period of one year beginning from the Commencement Date.
PHYSICIAN means a licensed medical practitioner bearing valid inforce registration with the Pakistan
Medical and Dental Council (PMDC), and who: is someone other than the Member or a relative of the
Member; is a Physician in an Approved Hospital; is not or never has been disqualified or license revoked
or suspended from practicing as a medical practitioner by the Pakistan Medical and Dental Council (PMDC)
or any statutory / regulatory / government body / entity, whether within Pakistan or outside Pakistan.
PRE-ADVICE means information provided by the Member or any of his family members accompanying him
at the hospital regarding his Hospitalization, through the Claims Helpline during his hospitalization.
Information should include details regarding:
a) Identification details of the patient to determine that he is the Member;
b) When Member was admitted;
c) Where Member is admitted;
d) Why Member is admitted (presenting complaints or diagnosis);
e) When Member is expected to leave hospital;
The Member must also advise Claims Helpline when he is discharged from Hospital and he should also
confirm or amend the diagnosis at that time as previously advised.
PRE-EXISTING CONDITIONS for the purposes of this Policy means any injury, illness, condition or
symptom: for which treatment, or medication, or advice, or diagnosis has been sought or received or was
foreseeable prior to the Commencement Date of the Policy for Member, or which originated or was known
by the Member to exist prior to the Commencement Date of this Policy, whether or not treatment, or
medication, or advice or diagnosis was sought or received.
SICKNESS means sickness or disease contracted for the first time after the Commencement Date.
TAKAFUL CONTRIBUTION means periodic contributions paid into the Group Family Participants Takaful
Fund for Takaful Death Benefit as well as Takaful Supplementary Benefits (if any).
WAQF DEED means the Deed of Waqf Settlement establishing the irrevocable Waqf Fund called EFU
Family Takaful Waqf Fund.
WAQF RULES means the Rules made under Waqf Deed related to the GFPTF. The Waqf Deed and the
Waqf Rules shall collectively be called, in this document, as Waqf Rules. The Waqf Rules are available at
the Head Office of the Window Takaful Operator.
WINDOW TAKAFUL OPERATOR’S FEES means the fees required to cover expenses of underwriting,
administration and general management of the GFPTF.
Words importing the singular number include the plural number and vice versa and words of masculine
gender shall include the feminine unless the context otherwise requires.
2. MASTER CONTRACT
This document, the Takaful Supplementary Benefit document(s), the Application Form (including the
Proposal Form), the PSS and any endorsements and documents that evidence the basis for, and any future
changes in, the aforesaid document(s) executed between the Member and the Window Takaful Operator,
together constitute the Contract. All statements and declarations made by the Member or Bank shall in the
absence of fraud, be deemed representations and not warranties and no such statement shall void the
Policy or be issued in defence of a claim hereunder unless it is contained in the Proposal and Declaration
thereof.
No one except an Authorized Signatory is authorized to extend the time for Contribution payment, to waive
any lapse or forfeiture, to waive any of the Window Takaful Operator's rights or requirements or to bind the
Window Takaful Operator by making any promise or by accepting any representation or information not
contained in the Proposal and Declaration for this Policy. The Window Takaful Operator shall not be bound
by any promise or representation heretofore or hereafter given by any person other than the authorized
representative and such approval be endorsed hereon.
3. AMENDMENT OF THIS POLICY
This Policy may be amended or changed at any time, without the consent of the Member covered
hereunder, on written request made by the Bank and agreement by the Company. Unless otherwise agreed,
any amendment or change to this Policy shall be binding on all Members whether covered under this Policy
prior to or on or after the date such amendment or change becomes effective.
4. ELIGIBILITY
The Eligible Person for this Policy are the present and future customers of the Bank who are within the
Eligibility Age range i.e. 18 to 64 years and who have given their valid consent to opt for family takaful
coverage under this Policy. Any Member shall not be eligible for multiple enrolments at a single point in
time. Eligibility requirements for spouses and children of the Insured Members will be as under.
• One spouse per Member within age limit of 18 to 65 years will be insured under the Policy.
• Children (unmarried) and economically dependent within age limit of minimum 06 months to 21
years.
5. EFFECTIVE DATE OF INDIVIDUAL FAMILY TAKAFUL COVERAGE
Customer will become eligible from the date on which the takaful contribution is paid for this takaful
coverage.
6. EVIDENCE OF AGE
Evidence of age of a Member satisfactory to the Window Takaful Operator will be required before any
benefit in respect of him is paid under this Policy and if after commencement of the Family Takaful Benefit
hereunder the date of birth of any Member is found to have been incorrectly notified to the Window Takaful
Operator, the Window Takaful Operator shall notify the Member and/or Bank of the adjustment to be made
under this Policy in respect of such incorrect notification. No benefit whatsoever shall be payable under the
Policy if the correct age of the Member and spouse is found to be more than maximum eligibility age of 66
years or 22 years in case of children (unmarried and economically dependent).
7. AMOUNT & CALCULATION OF CONTRIBUTION
Annual Takaful Contribution shall be payable to each Member as follows.

The Window Takaful Operator may however, by giving written notice to and taking consent of the Member,
modify rates for future Policy Period as it thinks fit.
The Window Takaful Operator reserves the right to charge any extra Contribution in respect of any Member
who engages in hazardous activities.
The Takaful Contributions would be deposited into a pool called the GFPTF.
Window Takaful Operator’s Fee: The Window Takaful Operator’s fee would be taken out by the Window
Takaful Operator from the GFPTF to cover for its expenses in underwriting, administering and general
management of the GFPTF.
8. BENEFITS
In consideration of the takaful contribution paid in accordance with Clause 7 of this Policy, upon receipt of
due proof in writing that the Member or Family Member has, whilst the Policy is in full force and effect,
suffered any of the event(s) set out below, the GFPTF managed by the Window Takaful Operator, subject
to the exclusions set out below and the terms and conditions contained herein, shall pay the following
Benefits:
8.1 Daily Hospital Cash Benefit:
If the Member or the Family Member, whilst the Policy is in full force and effect, as a result of Accident or
Sickness is necessarily confined as an Inpatient within a hospital, for at least 24 consecutive hours, under
the continuous attendance of a Physician, then the GFPTF managed by the Window Takaful Operator will,
upon the receipt of and due investigation of claim, pay the following Daily Benefit according to the chosen
category:

The Coverage for hospitalization due to Sickness under this policy takes effect after the elimination period
of 15 days from the enrolment date of Member under this Policy. There is no Elimination Period in case of
hospitalization due to an Accident.
No benefit is payable in case of confinement of less than 24 consecutive hours.
8.2 Daily ICU Confinement Benefit:
If the Member or the Family Member is necessarily confined as an Inpatient to Daily Intensive Care Unit
then an additional amount equal to 100% of the benefit amount mentioned in table provided in clause 9.1
would be paid.
8.3 Maximum Annual Benefit Limit:
Subject to other terms and conditions of this Policy, the Daily Benefit will be payable for maximum of 100
days under this Policy for all claims arising during one Policy Year.
8.4 Maximum Consecutive Hospitalization:
In case of consecutive hospitalization for more than 24 hours, the payment of Daily Benefit may continue
for a maximum of thirty (30) consecutive days.
8.5 Successive Periods of Hospital Confinement:
If within thirty (30) days of discharge following a period of Hospital confinement for which the Daily Benefit
is paid or payable under this Policy, the Member is readmitted and confined as an Inpatient, due to the
same or related causes, the Daily Benet will not be payable. If subsequent confinement as an Inpatient is
after a period of at least thirty (30) days from the date of discharge from the immediately preceding period
of Hospitalization, the confinement will be considered a new period of confinement and the Daily Benet will
be paid in accordance with the appropriate provisions of this Policy.
8.6 Accidental Medical Expense Reimbursement:
In the event where the Member or Family Member has, whilst the Policy is in full force and effect, incurred
medical expenses on account of treatment of injuries caused solely by violent, external and accidental
means and not as an indirect result of any fit, physical defect, illness or disorder, the GFPTF managed by
the Window Takaful Operator will pay an actual reimbursement amount up to the maximum limit assigned
for each family as per the selected plan category to Member in addition to any benefit under the policy:

8.7 Funeral Expense Benefit:
On death of the Member or Spouse, whilst the Policy is in full force and effect, the GFPTF managed by the
Window Takaful Operator will pay the amount specified in table below as per the selected plan category by
the Member.

8.8 Caesarian Section Surgery Benefit
If the female Member or Spouse, while insured in this Policy, has gone through Caesarean Section Surgery,
the GFPTF managed by Window Takaful Operator will pay the amount specified in table below as per the
selected pan category by the Member.

9. CESSATION OF MEMBERSHIP
Family Takaful Cover shall cease on event of any of the following:
a) The Member’s death.
b) Closing of account / membership or cancellation / cessation of the scheme by the Bank.
c) Member and spouse having attained the age of 66 years or children aged 22 years (unmarried and
economically dependent)
d) Non-payment of Takaful Contribution by the Member when due.
e) Member cancels his takaful coverage for a full refund of takaful contribution paid within fourteen
(14) days of Commencement Date by notifying for cancellation, in writing or by recorded telephone
call and provided that no claim has been filed under this Policy. No takaful contribution refund shall
be allowed in the event if any claims is filed under this Policy irrespective whether the claim is paid
or rejected by the Window Takaful Operator;
f) Any other date on which the Member ceases to be eligible for takaful cover for any fraudulent or
criminal reason affecting the takaul cover hereunder. Decision of the court shall be final in such
cases.
10. EXCLUSIONS
No benefit will be paid if the death or injury of the Member results directly, wholly or partly, as a result of or
related to:
a) Any pre-existing condition that includes disease and incapacity or bodily injury which existed before
this coverage.
b) Suicide and attempt to suicide, murder, self-inflicted injury, participation in criminal act or violation
of law and illegal act of the covered member.
c) Failure to seek or follow medical advice, taking of alcohol or drug.
d) War, invasion, act of a foreign enemy, hostilities (whether war be declared or not), armed or
unarmed truce, civil war, mutiny, rebellion, revolution, insurrection by military or usurped power,
riot or civil commotion, an illegal organization, or an industrial dispute.
e) Dental related treatments.
11. CANCELLATION
This Policy may be cancelled by the Member within 14 days from the Commencement Date. In this case,
the amount equal to the contribution shall be paid to the Member from GFPTF as settlement benefit of the
policy.
12. ASSIGNMENT
The Family Takaful Cover provided for under this Policy and the benefits payable hereunder are not
assignable.
13. RECORDS
The Bank shall keep a record of the insured Members containing, for each Member, the essential particulars
of the Assurance. This Policy gives the Company, by its duly authorized representative, the right at
reasonable times to inspect all books and records of the Bank relating to the assurance effected hereunder.
14. CLAIMS
The Bank shall notify the Window Takaful Operator of the claim event of any Member and furnish the
Window Takaful Operator, at its own expense, with all information necessary to determine whether the
benefit is payable in respect of that Member and the amount of the benefit.
The Takaful coverage affected hereunder shall carry no paid-up / surrender value.
Claim will be processed by the Window Takaful Operator following the submission of the claim documents
to the Window Takaful Operator or an entity authorized by the Window Takaful Operator.
Upon satisfaction, the GFPTF managed by the Window Takaful Operator shall pay the benefit amount.
15. TERMINATION OF THIS POLICY
The Window Takaful Operator reserves the right to terminate the Policy by giving 90 days’ notice. It is
clarified that any termination shall not affect the obligations of Bank and Window Takaful Operator assumed
at any time prior to the effective date of termination. Further, the Window Takaful Operator reserves the
right to give Bank three months’ written notice to add, alter or repeal the terms and conditions of the Policy
hereof.
Notwithstanding anything to the contrary in this Policy, the termination of this Policy shall have the following
effect:
a) No individual enrollment shall be entertained under this Policy after the time of termination.
16. LAW
The policy is governed by and interpreted according to the laws of Islamic Republic of Pakistan.
17. STATUTORY FUND
Under the provisions of the Insurance Ordinance 2000 and Takaful Rules 2012 this policy and any
additional benefits shall be referable to the Family Takaful Protection Business Statutory Fund of the
Window Takaful Operator. The Window Takaful Operator may by endorsement to the Policy change the
Statutory Fund(s) to which the policy and any additional benefits are referable.
