1.Project1. Project organisation for health and safety control

This risk assessment and method statement is for employees and sub-
contractors working for Brighouse Patent Walling Systems Ltd (BPWS). It
relates to the way in which telescopic handling is to be carried out, in line with
the company’s Health and Safety Policy.

All operatives must be fully trained in the work they are doing. They can
contact Daniel Cryer at any time with regards to any issues that may arise.

All fork lift operations will be overseen by Daniel Cryer. All employees and
sub-contractors report to him and are expected to report any health and safety
issues immediately.

All employees and subcontractors will receive a copy of this risk assessment
and method statement and will work in accordance to it.

1. Scope and method of work

The method statement relates to the safe way in which Stone masonry and
block work will be carried out in a safe and proper manner.

Method of Work

Stages of the work we will be undertaking

 Arrival on site.
 Get to place of work. The area will be assessed to ensure a safe route is
possible. Vehicles will be parked in the correct location and where
required High Visibility clothing will be worn at all times.
 Steel toe capped boots will be worn at all times.
 Any equipment and stone brought onto site will be unloaded in a safe
area and near the area of work.
 An area for waste will be established and any waste materials generated
will be kept in this area. Waste materials will be removed off site by and
agreed party unless it is agreed with the client that their waste facilities
can be used.
 Gloves will be worn when handling and transporting any materials.
 Foundations / footings may need to be dug. This will either be done
manually by spade or for large areas by mechanical digger. Only suitably
trained operatives will use mechanical equipment.

 Stones will be lifted into the correct position. Heavy or awkward shaped
stones will be lifted by two people.
 Stones may require some cutting. In which case a stihl saw is used. All
operatives will be suitably trained in using a stihl saw safely. Dust mask
and eye protection will be worn when using the saw.
 The job may require cement to be mixed. In which case gloves will be
worn. The cement will either be mixed manually or by machine. If a
machine is used then the operator will be suitably trained to operate it
safely.
 Once the job is completed all waste will be removed from site
 Any issues that arise during the job will be discussed before continuing.
 No work will be started if there is a risk to the health and safety of any
staff or subcontractors or any other contractors or visitors who maybe on
site.

3.Method3. Method Statement

HARD HAT, SAFETY BOOTS AND HIGH VISIBILITY JACKETS


MUST BE WORN AT ALL TIMES
 All personnel must be familiar with site safety rules, the traffic
management plan and accident procedures.
 After receiving instruction about works to be carried out, personnel will
proceed to the relevant work area.
 Personnel must refer to the risk assessment and ensure that all current
control measures are in place and continue to be in place for the duration
of the work task.
 Areas of work are to be kept clean and free from debris at all times both
during works and on completion of work tasks.
 On completion of work all tools and unused materials are to be returned to
the appropriate stores. The work area is to be left tidy and any waste
materials or debris placed into appropriate skips provided by BPWS.
 The area will be assessed to ensure a safe route to execute task is
possible.
 The correct PPE will be worn: high visibility jacket, steel toe capped boots,
hard hat, gloves, goggles and ear protection

The main hazards to your safety and health are;


a) Falling from height

Preventative Measures that must be taken;


a) Employees must be “competent” to carry out the task.
b) Employees must NOT carry out this task alone
c) Barriers erected at entrances and around the work area if deemed
necessary by the foreman to protect tenants.
d) Employees must not lift beyond your capabilities, get help if necessary.
e) Visitors and other members of staff are prohibited from entry unless
accompanied by competent person, all visitors issued with personal
protective equipment
f) You must read and be familiar with the Safety Data Sheets for any
COSHH that may be used these will contain information on first aid, fire
fighting, and accidental release measures.

Environmental Protection Measures you must take;


a) You must dispose of waste and spoil to the designated area or skip
provided for waste.

pedestrian you keep to any designated routes as indicated by the Principle
Contractor to reduce the risk of pedestrians impacting with vehicles and
items of plant machinery.

Personal Protective Equipment


Ensure that you are wearing the correct PPE; Hi-visibility waistcoats

Work Area (General Precautions)


• The work area is to be free of other trades working in the immediate
area.
• All debris, rubbish & trip hazards are to be removed from the work area
prior to the work commencing.
• If other trades are working within the property they must be advised as to
the work activities you are about to commence.

Maintenance and replacement of personal Protective equipment


Every employee or self-employed person shall ensure that any personal
protective equipment provided to them is maintained (including replaced or
cleaned as appropriate) in an efficient state, in efficient working order and
in good repair that they have received adequate training with regards to
the use of any particular item of Personal Protective Equipment.
• Self employed persons are to ensure they purchase and maintain PPE in
line with the relevant risk assessments
• Employed persons are to ensure they look after and use any PPE
provided for them and report any defects etc to their supervisor
immediately.

Welfare Facilities
Operatives will use the welfare facilities provided by BPWS to include toilet
facilities and Mess Canteen, the canteen facilities will be used for all meal
breaks and no breaks will be held in the work area, on any other part of
the site nor in vehicles (Private or company owned)

On Completion of Works
All work areas are to be left in a clean condition and all aspects of safety
with regards to the immediate work area and conduct a visual inspection

and proper manner before departing from the property you are working in.

Work Method
 The site manager must ensure that the telehandler to be used has been
maintained and has a current report of thorough examination covering
both the machine and any attachments.
 The site manager must ensure that the operator has proof of competence,
which should include proof of training (CPCS card or similar), proof of
familiarisation on the telehandler to be used and adequate experience of
the task to be undertaken.
 The supervisor must carry out a toolbox talk before work starts covering
the task to be carried out, personnel involved, work area, risks, exclusion
zones and procedures.
 At the start of each day or shift the operator should carry out the pre-use
checks specified in the manufacture's operating instructions.
 Seat belts must be worn at all times.
 The telehandlers brake engine must be switched off and the brake applied
before the operator leaves the cab. The key must be removed before any
personnel approach the telehandler to secure loads etc the operator must
ensure that the telehandler is made safe i.e. brake applied and engine
switched off.
 The operator should plan the route and final destination of the load,
making sure the route is clear of all obstacles, that there are no width or
height issues with the load or telehandler and that the final destination is a
safe and suitable place for the load. If the route involves travelling or
working on slopes, the manufacturer's operator’s instructions should be
consulted before traversing gradients and cross slopes.
 Before lifting a load the operator should get out and inspect the load -
check for warning signs, centre of gravity, loose materials on the load or
anything anchoring the load.
 Before lifting the load the operator should check ground conditions to
make sure it is suitable for them to drive and use stabilisers on. Spreader
mats should be used if the ground conditions or weight of the load make it
necessary to do so.
 The operator should then lift the load approximately 150mm (6 inches) and
check the load position, weight and balance. The load should then be
secured, strapped down or fastened where necessary.
 Before setting off, the boom should be telescoped in as far as possible and
the load lowered as close to the ground as possible to increase stability
whilst travelling.
 If the load obscures the operator’s view when travelling, a banksman
should be used. A banksman should also be used when reversing.

 Before placing the load in the final position the operator’s should check the
ground conditions.
 If the operator is unable to clearly see the load's final position, a qualified
banksman should be used to signal to the operator when landing the load.
 When lifting operation has been completed, the telehandler should be
stored in a safe place, on level ground where possible, with the handbrake
engaged, the boom and fork arms / handling attachment lowered to the
ground, the key removed and the cab locked. The key should be stored in
safe place and not left in machine.
 When using extension forks the safe working limit should be reduced as
the standard Rated Capacity chart will not give the correct information as it
is only applies to loads on standard length forks. Information on the
rated capacity of extended forks can be found in the manufacturer's
operating instructions.
 If the operator is unsure of any aspect of the lifting operation they must
immediately stop and consult their supervisor

There are a number of precautions that can reduce the risk of persons being
struck or the vehicle overturning. These are:

Visibility: Select telehandlers with the best view around them directly from the
driver position.

Rear visibility: Visibility to the rear may present a significant hazard. The
vehicle should be equipped with adequate aids so drivers can see areas
where people may be at risk. A signaller may be needed in some
circumstances.

Forward visibility: Similarly, when in operation with the boom raised and other
configurations, there may be a significant ‘blind spot’ to the front right-hand
side of the vehicle.

Ground conditions: Working on sloping, uneven or unstable ground can be


hazardous. Telehandlers normally require prepared, flat, graded surfaces to
operate safely. Even rough-terrain lift trucks have strict operational limits that
need to be observed.

Loading: Overloading can be prevented by selection of the correct vehicle and


good management. Moving with a raised load is dangerous and should be
avoided at all times.

Speed: a site speed limit should be established. Driving at excessive speed


around corners can cause the vehicle to overturn.

Training and competence

Drivers and pedestrians must be trained and competent regarding the


telehandler hazards and precautions:

Drivers should be trained, competent and authorised to operate the specific
telehandler used. Training certificates from recognised schemes help
demonstrate competence and certificates should be checked for validity; and

Pedestrians should be instructed in safe pedestrian routes on site and the


procedure for making drivers aware of their presence.

All personnel will be sufficiently trained and have the relevant qualifications to
carry out the task at hand

Inspection and maintenance

A programme of daily visual checks, regular inspections and servicing


schedules should be established in accordance with the manufacturer’s
instructions and the risks associated with each vehicle.

Drivers should be encouraged to report defects or problems. Reported


problems should be put right quickly and the machine taken out of service if
the item is safety critical.

The method statement should include:


the load description;

checks before, during and after lifting;

information about pick up and delivery points;

sequence of operations;

an assessment of the ground – is it suitable for the lifting and travelling


operations?

any exclusion zones?

training for operator, banksman and signaller;

arrangements for adequate supervision of operations;

method for communicating safe method of work;

what are the arrangements for ensuring that telehandler is fit for purpose?
