In accordance with the provisions outlined in the Toll Concession Agreement,

specifically Annex J, this is a formal reminder regarding the standardized format for
documenting and reporting road accidents occurring within your jurisdiction (see
attached Annex A).

The prescribed format under Annex J is crucial to ensuring uniformity, accuracy,


and transparency in the collection and reporting of road accident data. It is our
understanding that this format includes, but is not limited to, the following

b) Traffic volume
2) Balance Sheet Information
a) Book value of works

v) Non-Construction capitalized cost
b) Financial Data
i) Current Assets
ii) Liabilities
iii) Equity
c) Cashflow Information
3) [Additional required data point, etc.]

requirement in all submissions related to road accident incidents. Adhering to the
