AFFIDAVIT OF LEGITIMATION

THAT We, Spouses SAN PABLO V. BUOT AND


FRANCISCA O. AM-IS, both of legal age, Filipino citizens,
husband and wife, with residence and postal address at
Purok Talisay, Barangay Tandubuay, San Pablo, Zamboanga
del Sur, after having been duly sworn to in accordance with
law, do hereby deposes, states and declares:

1. That We are the same and identical persons as


spouses SAN PABLO V. BUOT AND FRANCISCA O. AM-IS
who got married on 23 August 2002, at Office of the
Municipal Mayor, San Pablo, Zamboanga del Sur, as
evidenced by our Certificate of Marriage issued by Local Civil
Registrar (LCR), Municipality of San Pablo, Zamboanga del
Sur, which is hereto attached and marked as Annex “A”
forming integral part hereof;

2. That I, SAN PABLO V. BUOT, was born on 29 June


1963, at Barangay Tandubuay, San Pablo, Zamboanga del
Sur, while I FRANCISCA O. AM-IS, was born on 13
December 1972, at Cangmunag, San Juan, Sequijor,
respectively;

3. That before we got married as above-stated, we


begot a child in the person of ZORAINE AM-IS, who was
born on 21 January 1996, at Barangay Tandubuay, San Pablo,
Zamboanga del Sur, as evidenced by her Certificate of Live
Birth which is hereto attached and marked as Annex “B”
forming integral part hereof;

4. That during our marriage, we were both single and


unmarried, as evidenced by our respective Certificate of No
Marriage (CENOMAR) which are hereto attached and marked
as Annexes “C and D”, respectively;

5. That we wish to legitimate our aforenamed daughter


so that she will become legitimated in her Certificate of Live
Birth and she could use her mother’s middle name of AM-IS
and her father’s family name or surname of BUOT or the full
name as ZORAINE AM-IS BUOT;
6. That we are executing this affidavit freely and
voluntarily to attest the veracity of the foregoing facts and
for whatever legal purposes and intent this affidavit may
serve it best.

IN WITNESS WHEREOF, We have hereunto set our


hands this 14th day of November, 2024, at Pagadian City,
Philippines.
