A COE can only be issued once and only a Certhed rue copy shall be
issued 1 case of loss. A certiied true copy of the COE shail ony be
issued wice per term except for special cases.

1. Secure an Affidavit of Loss from tre Public Attomey Office

(PAO) stating the reason for loss.

2. Present Affidavit of Lous to the Registrars Otice.

3. Keep the certified rue copy of COE sate and untampered.

Changing of subjects is only allowed # the subject is dissolved No

of classes during the semester oF after one(1) week from the opening

1. Seek the Dropping Form trom the Office of the University

Regetar.
2. Seek approval of the Sbject Professor
3. Submit the accomplished form to tre Office 0! the University

A student who withdrawn he/her regstraton within specified

University Regstrar
