CONSTRUCTION SAFETY AND HEALTH PROGRAM (CSHP) FOR THE CONSTRUCTION OF
PRIVATE RESIDENCE OR SMALL COMMERCIAL CONSTRUCTION PROJECTS
NAME: ZOSIE M. AGRIPO
LOCATED AT BRGY. STA. IRENE, BAYUGAN CITY, AGUSAN DEL SUR
1.0 Commitment to comply on OSH
I/We ZOSIE M. AGRIPO do hereby commit and bind myself/ourselves to comply with the
applicable provisions of the Occupational Safety and Health Standards (OSHS) governing Occupational
Safety and Health in the Construction Industry.
I/We hereby commit to implement a suitable Construction Safety and Health Program in the
construction of my/our residential house/small commercial building.
I/We also acknowledge our responsibilities to provide the appropriate personal protective
equipment (PPE) and job safety and health instructions and training to all our workers during the
duration of the construction.
2.0 Safety and Health Policy
In order to ensure the safety and health of the workers hired to construct my/our residential
house/ in the safest manner possible, I/We will implement the following:
a. Hazard Identification and Control Measures
Identify the potential hazards that may arise during the construction and provide the corresponding
safety measures to avoid possible exposure to these hazards:
Physical Hazards
Physical hazards are the most common in most workplaces. The physical hazards that i/we
identified for this project include but are not limited to the following: (please check on the box all
that applies)
□ machineries □ power and hand tools
□ electrical □ ladders and scaffolds
□ noise □ ventilation
□ exposure to heat □ tripping
□ fall hazards □ collapse
□ others (please specify) _______________
Identified hazard Control Measures
machineries To ensure machinery is safe you should check the machine is well maintained and fit to be used. Make sure
it is appropriate for the job, working properly and that all the safety measures are in place. Examples of
safety measures include guards, isolators, locking mechanisms and emergency off switches.
electrical Using appropriate personal protective equipment, such as gloves and safety glasses, safeguards workers
from electrical hazards.
noise Wear sound proof earphones
exposure to heat Wear approapriate clothing & stay dehydrated
fall hazards Adequate lighting (minimize glare and contrast) Guardrails for raised floors, mezzanines and balconies.
Minimize environmental influences. Slip-resistant flooring and mats.
power and hand tools Disconnect tools when not using them, before servicing and cleaning them, and when changing
accessories such as blades, bits, and cutters. Keep all people not involved with the work at a safe distance
from the work area. Secure work with clamps or a vise, freeing both hands to operate the tool.
ladders and scaffolds Keep both feet on the ladder. Never put one foot on a rung and the other on a different surface. Be sure
supported scaffolds have completely planked platforms, proper access, complete guardrails and proper ties
to buildings, where required. Keep a scaffold's wood planks unpainted so any cracks will be visible.
collapse follow the standard of OSH interms of scaffolding
Use additional sheet if necessary
Chemical Hazards
Chemical hazards are present workers handle chemical preparations in any form (solid, liquid
or gas). Some are safer than others, however, some workers are more sensitive to chemicals,
even the common solutions causing illness, skin irritation or breathing problems.

The chemical hazards that we identified for this project include but are not limited to the
following: (please check on the box all that applies)
□ solvents □ paint products
□ acids □ cleaning products
□ acetylene □ propane
□ gasoline □ explosive chemical
□ welding fumes □ others (please specify)_________________
Identified hazard Control Measures
solvents Use personal protective equipment such as gloves, eye protection, boots, or any other barriers when
working with solvents.
acids Wear chemical splash goggles, chemical-resistant gloves, and a chemical-resistant apron whenever using
concentrated acids or acid solutions.
acetylene 1. Wear appropriate personal protective equipment (PPE) when handling acetylene, including
safety glasses, gloves, and flame-resistant clothing.
2. Store acetylene cylinders in an upright position and secure them to prevent them from falling
over. Keep them in a well-ventilated area away from heat sources and combustible materials.
3. Keep a fire extinguisher nearby when working with acetylene, and know how to use it in case of
an emergency.
4. Do not smoke or use open flames in the vicinity of acetylene cylinders or when working with
acetylene.
5. Never attempt to fill or refill acetylene cylinders yourself. Leave that job to trained acetylene
professionals.
gasoline Wear gloves, eye protection and face protection (as needed to prevent skin and eye contact with liquid).
Wash hands or liquid-contacted skin thoroughly after handling. Do not eat, drink or smoke when using this
product.
welding fumes Workers should position themselves to avoid breathing welding fume and gases. For example, workers
should stay upwind when welding in open or outdoor environments. . General ventilation, the natural or
forced movement of fresh air, can reduce fume and gas levels in the work area.
paint products Use Personal Protective Equipment (PPE),Ensure Proper Ventilation,Inspect and Maintain
Equipment,Practice Safe Ladder Usage,Prevent Slips and Trips,Handle Chemicals Safely,Use Adequate
Lighting& Minimize Lead Exposure.
Use additional sheet if necessary
Biological Hazards
Biological hazards come from working with infectious people, plants, and other living materials.
The biological hazards that we have identified for this project includes but not limited to the
following: (please check on the box all that applies)
□ blood or other body fluids □ fungi
□ bacteria and viruses □ plants
□ insect bites □ animal and bird droppings
□ others (please specify) __________________________
Identified hazard Control Measures
fungi used rubber boots & gloves in order to keep away from fungi specially
in wet areas
Insect bites wear long sleeve shirt and long pants
Use additional sheet if necessary
Ergonomic Hazards
Ergonomic hazards occur when the type of work, body position and working conditions put
strain on the body. They are the hardest to spot since one does not immediately notice the
strain on your body or the harm these hazards pose. Short-term exposure may result in "sore
muscles" on the days following exposure, but long-term exposure can result in serious
musculoskeletal injuries.
The ergonomic hazards that we identified for this project includes but not limited to the
following: (please check on the box all that applies)
□ poor lighting □ frequent lifting
□ poor posture □ repetitive motion
□ exertion of force □ awkward movement
□ others (please specify) _______________________
